This Purchase Order authorizes the procurement of licensed Social Worker professional services to be delivered within the jurisdiction of United States San Francisco. The contracted Social Worker shall provide comprehensive case management, family intervention, crisis counseling, and community resource coordination for underserved populations in the San Francisco Bay Area. The Social Worker must hold a valid California Licensed Clinical Social Worker (LCSW) credential issued by the California Board of Behavioral Sciences and maintain a minimum of five (5) years of post-licensure clinical experience in urban community settings.
The Social Worker shall report directly to the Community Wellness Program supervisor and operate in accordance with all applicable federal, state, and local regulations governing social work practice in the State of California. All services rendered under this Purchase Order shall be conducted at the designated facility in United States San Francisco or at approved community outreach sites within San Francisco County, as directed by the purchasing entity.

- Payment Terms: Net 30 days from receipt of approved invoice. Payment shall be processed via electronic funds transfer (EFT) to the account designated by the supplier. All invoices must reference this Purchase Order number (SF-SW-2025-04872) and include a detailed breakdown of services rendered by the Social Worker during the billing period.
- Service Period: This Purchase Order covers a twelve (12) month engagement period commencing July 1, 2025, and concluding June 30, 2026. Renewal is subject to mutual written agreement and availability of appropriated funds within the United States San Francisco municipal budget.
- Performance Standards: The Social Worker shall maintain a caseload not exceeding twenty-five (25) active clients at any given time. Monthly progress reports shall be submitted to the Community Wellness Program supervisor by the fifth (5th) business day of each month. Failure to meet performance benchmarks for two consecutive months shall constitute grounds for termination of this Purchase Order with thirty (30) days written notice.
- Compliance: The Social Worker and the supplier shall comply with all applicable federal laws, California state regulations, and San Francisco municipal ordinances, including but not limited to the California Social Work Practice Act (Business and Professions Code Sections 4900-4999.10), HIPAA privacy requirements, and the San Francisco Fair Chance Ordinance.
- Insurance: The supplier shall maintain professional liability insurance with a minimum coverage of $1,000,000 per occurrence and $2,000,000 aggregate, naming the San Francisco Department of Public Health as an additional insured party. Certificates of insurance shall be provided prior to the commencement of services.
- Confidentiality: All client records, case files, and personally identifiable information handled by the Social Worker shall be maintained in strict accordance with California Confidentiality of Medical Information Act (CMIA) and applicable federal privacy statutes. No client information shall be disclosed without proper authorization.
- Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. The purchasing entity reserves the right to terminate immediately for cause, including but not limited to breach of contract, loss of Social Worker licensure, or failure to maintain required insurance coverage.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the State of California. Any disputes arising hereunder shall be resolved through binding arbitration in United States San Francisco, in accordance with the rules of the American Arbitration Association.
- Non-Collusion: The supplier certifies that the pricing submitted in response to this Purchase Order was developed independently and without collusion, consultation, communication, or agreement with any other bidder or competitor regarding pricing or terms.
- Anti-Discrimination: The Social Worker shall provide services in a non-discriminatory manner in compliance with the San Francisco Human Rights Ordinance and all applicable federal civil rights legislation. Services shall be accessible to all residents of United States San Francisco regardless of race, ethnicity, gender identity, sexual orientation, disability, or immigration status.
This Purchase Order is authorized for execution by the undersigned officials of the San Francisco Department of Public Health.
