- This Purchase Order is issued in accordance with the Australian Consumer Law (2010) and the Victorian Health Services Act 1988. All services rendered by the Speech Therapist shall comply with the professional standards set by Speech Pathology Australia and the National Registration and Accreditation Scheme (NRAS) where applicable.
- The Speech Therapist shall maintain current professional indemnity insurance with a minimum coverage of AUD $10,000,000 and provide a certificate of insurance prior to the commencement of services in Australia Melbourne.
- All Speech Therapist services shall be delivered in accordance with the Australian Privacy Principles (APPs) under the Privacy Act 1988 (Cth). Client records, assessment data, and progress documentation shall be stored in compliance with Victorian health records legislation.
- The Supplier acknowledges that this Purchase Order constitutes a binding agreement for the provision of Speech Therapist services within the Australia Melbourne metropolitan area, including the City of Melbourne, City of Yarra, City of Boroondara, and City of Port Phillip local government areas.
- Payment shall be made within thirty (30) calendar days of receipt of a valid tax invoice. Invoices must reference this Purchase Order number (PO-2025-MEL-04872) and include the Supplier's ABN. Payment shall be made via electronic funds transfer (EFT) to the bank account nominated by the Supplier.
- The Speech Therapist shall provide a written progress report to the Buyer's Clinical Operations Director on a quarterly basis. Failure to submit reports within fourteen (14) days of the quarter-end date may result in a service credit of 5% of the quarterly invoice value.
- Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Buyer shall pay for all Speech Therapist services rendered up to the effective date of termination.
- The Supplier warrants that the Speech Therapist engaged under this Purchase Order holds a minimum of a Master's degree in Speech Pathology from an Australian university and is a current member in good standing with Speech Pathology Australia.
- All disputes arising from this Purchase Order shall be resolved in accordance with the Commercial Arbitration Act 2011 (Vic) and shall be heard in the courts of Victoria, Australia Melbourne.
- This Purchase Order shall be governed by and construed in accordance with the laws of the State of Victoria, Australia Melbourne, and the Commonwealth of Australia.
The Speech Therapist services outlined in this Purchase Order shall commence on 1 July 2025 and conclude on 30 June 2026. Sessions shall be conducted Monday through Friday between the hours of 8:30 AM and 5:30 PM Australian Eastern Standard Time (AEST), at the Buyer's premises located in Australia Melbourne or via approved telehealth platforms as specified in Line Item 04. The Speech Therapist shall adhere to the Australian National Disability Insurance Scheme (NDIS) practice standards where services are NDIS-funded.
