This Purchase Order is issued by Doha Integrated Health & Rehabilitation Centre to formally engage the services of a qualified Speech Therapist to deliver comprehensive speech-language pathology services to patients and clients within the Doha, Qatar facility. The Speech Therapist shall be responsible for the assessment, diagnosis, treatment planning, and ongoing therapeutic intervention for individuals presenting with speech, language, voice, swallowing, and cognitive-communication disorders. All services under this Purchase Order shall be rendered exclusively at the buyer's premises located in Al Sadd District, Doha, Qatar, unless otherwise authorized in writing by the Director of Clinical Services.

Commencement Date: The Speech Therapist shall commence duties no later than 1 August 2025 at the Doha, Qatar facility. A written confirmation of the assigned Speech Therapist's credentials, registration, and availability must be submitted to the buyer at least fourteen (14) calendar days prior to the commencement date.
Qualification Requirements: The Speech Therapist engaged under this Purchase Order must hold a Master's degree or equivalent in Speech-Language Pathology, be registered with the Qatar Health Professions Council (QHPC), and possess a valid practice license for the State of Qatar. Proof of registration shall be provided prior to the first day of service.
Payment Terms: Payment shall be made on a monthly basis within thirty (30) days of receipt of a valid tax invoice. The supplier shall submit invoices by the 5th of each month for services rendered in the preceding month. Late payments shall accrue interest at the rate of 1.5% per month as permitted under Qatar commercial law.
Working Hours and Location: The Speech Therapist shall work Monday through Thursday, 8:00 AM to 4:30 PM, and Friday 8:00 AM to 12:00 PM, in accordance with the standard working week in Qatar Doha. All clinical services shall be delivered at the buyer's Al Sadd District facility unless a home-visit or telehealth session is specifically authorized in writing.
Confidentiality and Data Protection: The Speech Therapist and the supplier shall comply with all applicable data protection and patient confidentiality laws in Qatar, including the Qatar Personal Data Protection Law (Law No. 13 of 2016). All patient records shall be stored in the buyer's electronic health record system and shall not be removed from the Doha premises.
Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the buyer shall pay for all services rendered up to the effective date of termination. The supplier shall ensure a smooth transition of all active patient caseloads to a replacement Speech Therapist approved by the buyer.
Compliance with Local Regulations: The supplier and the Speech Therapist shall comply with all regulations issued by the Qatar Ministry of Public Health, the Qatar Health Professions Council, and any applicable municipal or labor laws in Doha, Qatar. The supplier shall maintain all required insurance coverage, including professional indemnity insurance with a minimum cover of QAR 1,000,000.
Dispute Resolution: Any dispute arising from this Purchase Order shall first be subject to good-faith negotiation. If unresolved within thirty (30) days, the dispute shall be referred to arbitration in Doha, Qatar, in accordance with the rules of the Qatar International Arbitration Centre (QIAC). The governing law shall be the laws of the State of Qatar.

Note: This Purchase Order is subject to the buyer's internal procurement policy and the Qatar Government Procurement Law where applicable. All services described herein are to be performed in the State of Qatar, with the primary site of delivery being Doha. The Speech Therapist must be physically present in Qatar Doha for all in-person clinical sessions and must hold a valid Qatar residence permit (QID) for the duration of the engagement.

By signing below, both parties acknowledge and agree to the terms, conditions, scope of work, and pricing outlined in this Purchase Order for the engagement of a Speech Therapist in Qatar Doha. This document constitutes a binding agreement upon execution by both authorized representatives.
