This Purchase Order is issued by Shree Lakshmi Multispecialty Hospital Pvt. Ltd., India Mumbai, for the engagement of a qualified and registered Surgeon to provide comprehensive surgical and perioperative services within the hospital's surgical wing. The Surgeon shall be responsible for performing elective and emergency surgical procedures, conducting pre-operative assessments, managing post-operative patient care, supervising junior surgical residents, and participating in multidisciplinary tumor board meetings as required by the hospital's clinical governance framework.

The Surgeon engaged under this Purchase Order must hold a valid Medical Council of India (MCI) / National Medical Commission (NMC) registration, a Master of Surgery (MS) degree in General Surgery, and a Fellowship or MCh in a recognized surgical subspecialty. The Surgeon shall comply with all applicable regulations set forth by the Maharashtra State Medical Council and the Central Council of Indian Medicine operating within India Mumbai.

  1. Commencement: The Surgeon shall commence duties under this Purchase Order on 1 August 2025 at the India Mumbai facility located at Andheri East. A formal orientation and credentialing process shall be completed within the first seven (7) calendar days.
  2. Duration: This Purchase Order is valid for a period of twelve (12) months from the date of commencement, subject to mutual renewal by written agreement between the parties.
  3. Payment Terms: Payment shall be made on a monthly basis within fifteen (15) days of the Surgeon's submission of a detailed service report. All payments shall be processed via NEFT/RTGS to the Surgeon's designated bank account in India Mumbai. A 18.5% GST shall be applicable on the total contract value as per the Goods and Services Tax Act, 2017, applicable in India Mumbai.
  4. Compliance: The Surgeon shall maintain all necessary professional registrations, malpractice insurance (minimum coverage of INR 50,00,000), and adherence to the hospital's clinical protocols. The Surgeon shall comply with all data protection requirements under the Digital Personal Data Protection Act, 2023, as applicable in India Mumbai.
  5. Confidentiality: The Surgeon shall maintain strict confidentiality regarding all patient records, hospital operational data, and proprietary surgical techniques. This obligation shall survive the termination of this Purchase Order for a period of five (5) years.
  6. Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of gross professional misconduct, the Buyer may terminate immediately without notice, in accordance with the Medical Council of India's Code of Ethics.
  7. Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of India, with jurisdiction vested in the competent courts of India Mumbai, Maharashtra.
  8. Dispute Resolution: Any dispute arising out of this Purchase Order shall first be attempted to be resolved through mediation. If unresolved within 30 days, the matter shall be referred to arbitration under the Arbitration and Conciliation Act, 1996, with the seat of arbitration in India Mumbai.

This Purchase Order constitutes a formal and binding procurement document. By signing below, both parties acknowledge and agree to all terms, conditions, and service specifications outlined in this Purchase Order for the engagement of the Surgeon at the India Mumbai facility.
