This Purchase Order is issued by the Singapore Singapore National Healthcare Authority to formally engage the services of a qualified Surgeon for the provision of specialized surgical procedures, pre-operative consultations, and post-operative care within the jurisdiction of Singapore Singapore. The Surgeon engaged under this Purchase Order shall be a fully licensed and registered medical practitioner holding valid credentials issued by the Singapore Singapore Medical Council and shall demonstrate a minimum of fifteen (15) years of clinical surgical experience in the relevant specialty.
The scope of this Purchase Order encompasses the following surgical engagements to be performed at the Singapore Singapore Central Medical Centre, located at 75 Hospital Drive, Singapore Singapore 168607. The Surgeon shall be responsible for all aspects of surgical planning, execution, and follow-up care as outlined in the attached clinical protocol annexures. All procedures shall comply with the Singapore Singapore Health Services Act and the regulatory standards set forth by the Singapore Singapore Ministry of Health.

4.1 This Purchase Order is governed by the laws of Singapore Singapore and shall be interpreted in accordance with the Singapore Singapore Contracts (General Part) Act. Any disputes arising from this Purchase Order shall be resolved through arbitration in Singapore Singapore under the rules of the Singapore International Arbitration Centre (SIAC).
4.2 The Surgeon engaged under this Purchase Order shall maintain valid professional indemnity insurance with a minimum coverage of SGD 5,000,000 per claim, as mandated by Singapore Singapore healthcare regulations. Proof of insurance shall be submitted prior to the commencement of any surgical procedure.
4.3 Payment terms for this Purchase Order shall be structured as follows: thirty percent (30%) upon acceptance of this Purchase Order by the Surgeon, forty percent (40%) upon completion of all primary surgical procedures, and the remaining thirty percent (30%) upon final post-operative clearance and submission of all required documentation to the Singapore Singapore National Healthcare Authority.
4.4 The Surgeon shall adhere to all clinical protocols, patient safety standards, and ethical guidelines established by the Singapore Singapore Medical Council. Any deviation from the agreed surgical plan must be documented and approved in writing by the supervising medical director at the Singapore Singapore Central Medical Centre.
4.5 This Purchase Order is valid for a period of one hundred and eighty (180) days from the date of issue. Should the Surgeon fail to commence services within thirty (30) days of acceptance, this Purchase Order shall be deemed null and void, and the Singapore Singapore National Healthcare Authority reserves the right to reissue a new Purchase Order to an alternative qualified Surgeon.
4.6 All patient data and medical records generated under this Purchase Order shall be stored in compliance with the Singapore Singapore Personal Data Protection Act (PDPA). The Surgeon shall not disclose any patient information to third parties without explicit written consent from the patient and approval from the Singapore Singapore data protection officer.
By signing below, both parties acknowledge and agree to all terms, conditions, and financial obligations outlined in this Purchase Order for the engagement of the Surgeon in Singapore Singapore. This document constitutes a binding agreement between the Singapore Singapore National Healthcare Authority and the named Surgeon upon execution by both authorized representatives.
