- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Upon expiry, all chemist product and service prices may be revised to reflect current South African pharmaceutical market conditions.
- Payment terms: 50% deposit upon acceptance of this Quotation Estimate, with the remaining 50% payable within thirty (30) days of the final monthly invoice. All payments to be made in South African Rand (ZAR) via electronic funds transfer (EFT) to the registered bank account of Sandton Chemist & Pharmacy (Pty) Ltd.
- All chemist pharmaceutical products supplied under this Quotation Estimate are sourced from approved South African suppliers and comply with the Medicines and Related Substances Act (Act 101 of 1965) and all regulations enforced by the South African Health Products Regulatory Authority (SAHPRA).
- The chemist compounding services outlined in Item 02 are performed exclusively by registered pharmacists and chemist technicians holding valid South African professional registration numbers with the Health Professions Council of South Africa (HPCSA).
- Delivery of all chemist products within the Johannesburg metropolitan area (including Sandton, Braamfontein, Rosebank, Fourways, and Midrand) is included at no additional cost. Deliveries outside the Johannesburg metro area will be subject to a separate logistics Quotation Estimate.
- This Quotation Estimate does not constitute a binding contract until both parties have signed and returned the acceptance section below. The chemist service provider reserves the right to decline any portion of this Quotation Estimate if supply constraints arise due to South African pharmaceutical import regulations.
- All clinical chemist consultations and medication reviews conducted under this Quotation Estimate are subject to the client's medical aid scheme authorisation where applicable. The chemist service provider will submit all necessary documentation to the relevant medical aid fund in Johannesburg.
- Any disputes arising from this Quotation Estimate shall be resolved in accordance with the laws of the Republic of South Africa, and the courts of the Gauteng Division, Johannesburg, shall have exclusive jurisdiction.
- Confidentiality: All patient and corporate health data handled by the chemist service provider under this Quotation Estimate shall be protected in compliance with the Protection of Personal Information Act (POPIA), Act 4 of 2013, South Africa.
