This Quotation Estimate is issued by Shanghai Wellness & Nutrition Consulting Co., Ltd. to provide a comprehensive and transparent cost breakdown for professional Dietitian services to be delivered within the metropolitan area of China Shanghai. The scope of this Quotation Estimate covers a twelve-month (12-month) retainer engagement for a registered Dietitian to provide ongoing nutritional assessment, meal planning, dietary counselling, and corporate wellness programming for the client organisation. The Dietitian assigned to this engagement holds a Master's degree in Clinical Nutrition from Fudan University, China Shanghai, and is registered with the China Nutrition Society (CNS). The Dietitian possesses over eight years of professional experience in corporate wellness, sports nutrition, and chronic disease dietary management. All services are delivered in accordance with the National Health Commission guidelines and the Dietary Guidelines for Chinese Residents (2022 edition). The Dietitian is fluent in both Mandarin Chinese and English, ensuring seamless communication with the client's international and local staff members.
- 4.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be subject to revision based on prevailing market conditions in China Shanghai.
- 4.2 Payment terms: 40% advance payment upon acceptance of this Quotation Estimate, 30% upon completion of the sixth month, and the remaining 30% upon delivery of the annual report. All payments shall be made via bank transfer to the account of Shanghai Wellness & Nutrition Consulting Co., Ltd.
- 4.3 The client agrees to provide a minimum of seven (7) days' written notice for any rescheduling of Dietitian consultation sessions. Cancellations made within 48 hours of a scheduled session will be charged at 50% of the session fee.
- 4.4 All nutritional advice provided by the Dietitian is for general wellness and dietary guidance purposes and does not constitute a medical diagnosis or treatment. The Dietitian will coordinate with the client's attending physician in China Shanghai where clinical intervention is required.
- 4.5 Confidentiality: All personal health data, dietary records, and nutritional assessments collected by the Dietitian shall be treated as strictly confidential in compliance with the Personal Information Protection Law of the People's Republic of China (PIPL, effective 1 November 2021).
- 4.6 This Quotation Estimate does not include the cost of laboratory blood work, body composition scans, or any third-party medical tests. Such costs, if required, will be quoted separately and borne by the client.
- 4.7 Any additional services beyond the scope defined in this Quotation Estimate will be subject to a supplementary Quotation Estimate issued by the undersigned.
- 4.8 Governing law: This Quotation Estimate and any resulting contract shall be governed by and construed in accordance with the laws of the People's Republic of China. Disputes shall be resolved through arbitration at the Shanghai International Economic and Trade Arbitration Commission (SHIAC).
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined herein. Upon countersignature by both parties, this Quotation Estimate shall constitute a binding service agreement for the Dietitian engagement described above.
