This Quotation Estimate outlines the full scope of Dietitian consultation services, nutritional assessments, personalised meal planning, and ongoing dietary support that will be provided. All pricing is presented in Euros (EUR) and is inclusive of applicable Dutch VAT (BTW) at the standard rate of 21%. This document serves as a formal Quotation Estimate and does not constitute a binding contract until both parties have signed and returned the acceptance section at the end of this document.

The following Dietitian services are included in this Quotation Estimate for the Netherlands Amsterdam area:

• Initial Comprehensive Nutritional Assessment: A 90-minute in-person consultation at our Amsterdam clinic where our registered Dietitian will conduct a full dietary history, body composition analysis, blood work review, and lifestyle evaluation.
• Personalised Nutrition Plan Development: A customised, culturally adapted meal plan reflecting the diverse food landscape of the Netherlands, incorporating local Dutch, Indonesian, Surinamese, and international dietary preferences common in Amsterdam.
• Follow-Up Consultations: Four (4) subsequent 45-minute Dietitian follow-up sessions spread over a twelve-week period to monitor progress, adjust dietary recommendations, and address emerging nutritional concerns.
• Dietary Education Workshop: A one-hour group or private educational session covering topics such as sustainable eating, label reading, and managing common dietary conditions (diabetes, hypertension, food allergies, and intolerances).
• Ongoing Email & Phone Support: Unlimited email correspondence and two (2) thirty-minute phone consultations with the assigned Dietitian during the twelve-week programme period.
• Referral Coordination: If required, the Dietitian will coordinate with the client's general practitioner (huisarts) or specialist physician in the Netherlands Amsterdam healthcare network to ensure integrated care.

1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, prices may be subject to revision based on current market conditions in the Netherlands Amsterdam healthcare sector.
2. All Dietitian services described in this Quotation Estimate must be delivered by a registered Dietitian holding a valid diploma from a recognised Dutch institution (e.g., Vrije Universiteit Amsterdam, Universiteit van Amsterdam, or equivalent) and registered with the Dutch Dietitians Association (Nederlandse Vereniging voor Diëtisten).
3. Payment is due within thirty (30) days of the date of this Quotation Estimate or upon completion of services, whichever is later. Late payments will incur a statutory interest rate of 8% per annum in accordance with Dutch civil law (Burgerlijk Wetboek).
4. Cancellations of scheduled Dietitian appointments must be made at least forty-eight (48) hours in advance. Late cancellations or no-shows will be charged at 50% of the applicable session fee.
5. All personal health data collected during the Dietitian consultations will be processed in strict compliance with the European General Data Protection Regulation (GDPR) and the Dutch implementation act (UAVG). Data will be stored securely within the Netherlands Amsterdam premises and will not be shared with third parties without explicit client consent.
6. This Quotation Estimate covers the services listed in Section 3 only. Any additional Dietitian consultations, specialised testing, or extended programme durations beyond the twelve-week period will be subject to a separate Quotation Estimate and prior written approval.
7. In the event of force majeure or unforeseen circumstances preventing the delivery of services in the Netherlands Amsterdam area, both parties agree to negotiate in good faith to reschedule or adjust the scope of work.
8. This Quotation Estimate is governed by the laws of the Netherlands. Any disputes arising from this document shall be subject to the exclusive jurisdiction of the competent court in Amsterdam, Netherlands.

By signing below, the client acknowledges receipt of this Quotation Estimate and authorises Amsterdam Nutritional Health Centre to proceed with the Dietitian services as described herein. This signature converts this Quotation Estimate into a binding service agreement between both parties.
