This Quotation Estimate is issued by Colombo Wellness & Nutrition Centre (Pvt) Ltd., a registered and licensed healthcare facility operating in the heart of Sri Lanka Colombo, to provide a comprehensive and transparent breakdown of professional Dietitian services. This Quotation Estimate covers a twelve-month (12-month) engagement period during which the client will receive structured Dietitian consultations, personalised nutrition plans, and follow-up monitoring. All services are delivered at our primary clinic located on Galle Road, Colombo 03, Sri Lanka Colombo, or via telehealth consultation as specified in the service schedule below.

3.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, the Dietitian service rates and availability may be subject to revision based on prevailing market conditions in Sri Lanka Colombo.
3.2 Payment is due within fourteen (14) days of acceptance of this Quotation Estimate. A 50% advance payment is required to secure the initial Dietitian assessment appointment. The remaining balance shall be payable in two equal instalments at the six-month and twelve-month marks.
3.3 All Dietitian consultations are subject to a 24-hour cancellation notice. Cancellations made within 24 hours of the scheduled appointment will incur a 50% fee of the applicable session rate.
3.4 The Dietitian services described in this Quotation Estimate are for nutritional counselling and dietary management only. They do not constitute medical treatment, prescription of pharmaceutical drugs, or a substitute for consultation with a registered medical practitioner. In cases of acute medical emergencies, the client is advised to seek immediate care at the nearest hospital in Sri Lanka Colombo.
3.5 All personal health information, dietary records, and consultation notes are handled in strict confidence in accordance with the Sri Lanka Data Protection Act No. 9 of 2022. No information will be shared with third parties without the client's written consent.
3.6 This Quotation Estimate does not include the cost of laboratory investigations (blood tests, lipid profiles, HbA1c, etc.) unless specifically arranged through our in-house pathology partner. Such costs will be quoted separately.
3.7 In the event of force majeure, including but not limited to natural disasters, government-imposed restrictions, or public health emergencies affecting Sri Lanka Colombo, the Dietitian service schedule may be temporarily modified to telehealth delivery without additional charge.
Note: This Quotation Estimate reflects the standard Dietitian service package for individual clients residing in or regularly visiting Sri Lanka Colombo. Corporate wellness packages, multi-family plans, and institutional contracts (hospitals, schools, corporate offices in Colombo) may qualify for volume-based discounts of up to 15%.
