1. Purpose of This Quotation EstimateThis Quotation Estimate has been prepared by NutriCare Zurich GmbH to provide a comprehensive and transparent financial overview of the professional Dietitian services to be delivered to the client within the city of Switzerland Zurich and its surrounding metropolitan area. This document outlines the scope of work, itemized costs, payment terms, and conditions governing the engagement. The Dietitian services described herein are designed to meet the specific nutritional and dietary requirements of the client's workforce and stakeholders, in full compliance with Swiss federal health regulations and the professional standards set by the Swiss Society for Nutrition (SGE/SSN).
2. Scope of Dietitian ServicesThe following Dietitian services are included in this Quotation Estimate for delivery in Switzerland Zurich:
- Initial Nutritional Assessment: A comprehensive one-on-one consultation conducted by a certified Dietitian at our Zurich office or at the client's premises. This includes a full dietary history, body composition analysis, blood work interpretation, and identification of nutritional deficiencies or imbalances.
- Personalized Meal Planning: Development of customized, culturally appropriate meal plans tailored to the client's dietary preferences, medical conditions, and lifestyle. All plans will incorporate locally sourced ingredients available in the Zurich market, reflecting Swiss culinary traditions while adhering to evidence-based nutritional science.
- Ongoing Dietary Counseling: Regular follow-up sessions (bi-weekly) with the assigned Dietitian to monitor progress, adjust meal plans, and provide continuous guidance. Sessions may be conducted in person at our Zurich clinic or via secure video consultation.
- Corporate Nutrition Workshops: Two half-day workshops per quarter delivered at the client's Zurich office, covering topics such as sustainable eating, managing stress-related nutrition, and workplace wellness strategies. Each workshop is led by the senior Dietitian and accommodates up to 30 participants.
- Specialized Dietary Management: Ongoing support for clients managing specific conditions including diabetes, cardiovascular disease, food allergies, and weight management, all handled by a qualified Dietitian registered with the Swiss professional body.
- Nutritional Reporting: Monthly progress reports and quarterly summary documents provided to the client's management team, detailing adherence rates, health metric improvements, and recommendations for the upcoming period.

Payment for the services outlined in this Quotation Estimate shall be made in monthly installments of CHF 6,192.75 (inclusive of VAT), due on the 5th business day of each month via bank transfer to the NutriCare Zurich GmbH account at UBS AG, Zurich. The first installment is due upon acceptance of this Quotation Estimate. A 1.5% monthly late payment interest will apply to overdue balances in accordance with Swiss Code of Obligations (OR Art. 104).
5. Terms and Conditions- This Quotation Estimate is valid for 30 calendar days from the date of issue. After the expiry date, pricing may be subject to revision based on market conditions in Switzerland Zurich.
- All Dietitian services are to be performed by qualified professionals registered with the Swiss Society for Nutrition and holding a valid professional license issued by the Canton of Zurich.
- The Dietitian shall maintain strict confidentiality in accordance with Swiss Federal Data Protection Act (DSG) and professional ethical codes. All client health data shall be stored securely within Swiss borders.
- Services are to be delivered primarily at the client's premises in Switzerland Zurich or at the NutriCare Zurich clinic. Travel beyond the Zurich metropolitan area (Zürichsee region) will be billed at CHF 1.20 per kilometer.
- Either party may terminate the engagement with 30 days' written notice. In the event of early termination, a pro-rata charge for services already rendered will apply.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties. Acceptance shall be confirmed via signed copy of this document or a formal purchase order referencing Quotation Estimate No. QE-2025-04871.
- All disputes arising from this engagement shall be resolved under the jurisdiction of the courts of the Canton of Zurich, Switzerland, in accordance with Swiss law.
- The Dietitian services provided are for nutritional guidance and do not replace medical treatment. In cases of acute medical conditions, the Dietitian will refer the client to an appropriate physician in Switzerland Zurich.
