1. INTRODUCTION AND SCOPE

Nutrivita Health Consultancy Ltd is pleased to submit this Quotation Estimate for the provision of comprehensive Dietitian services in Uganda Kampala. This Quotation Estimate has been prepared in response to your request for a qualified and registered Dietitian to deliver nutritional assessment, dietary counselling, and community nutrition intervention programs across selected health facilities in the Kampala Capital City Metropolitan Area.

Our firm employs a team of certified Dietitians who are registered with the Uganda Dietitians Association (UDA) and the Health Professions Council of Uganda (HPCU). The Dietitian services outlined in this Quotation Estimate are designed to address malnutrition, obesity, diabetes management, maternal and child nutrition, and corporate wellness programs within the Uganda Kampala region. All services will be delivered in accordance with the Uganda National Nutrition Policy and the guidelines set forth by the Ministry of Health, Republic of Uganda.

3. TERMS AND CONDITIONS OF THIS QUOTATION ESTIMATE

This Quotation Estimate is valid for thirty (30) days from the date of issue. After the expiry date, prices may be subject to revision due to market fluctuations in Uganda Kampala.
All Dietitian services shall be delivered by qualified professionals registered with the Health Professions Council of Uganda and the Uganda Dietitians Association.
Payment terms: Fifty percent (50%) advance payment upon acceptance of this Quotation Estimate, with the remaining fifty percent (50%) payable upon completion of the contracted services.
Payment shall be made in Ugandan Shillings (UGX) via bank transfer to Nutrivita Health Consultancy Ltd, Stanbic Bank, Account No. 010-452-889-001, Kampala Branch.
All services will be conducted within the Uganda Kampala metropolitan area. Any travel beyond Kampala District boundaries will be quoted separately.
The Dietitian team shall adhere to all data protection regulations as stipulated by the Uganda Data Protection and Privacy Act, 2019, when handling patient nutritional records.
Any changes to the scope of Dietitian services outlined in this Quotation Estimate must be agreed upon in writing by both parties before implementation.
This Quotation Estimate does not constitute a binding contract until a formal service agreement is executed by both parties.
Disputes arising from this Quotation Estimate shall be resolved through mediation in Kampala, Uganda, in accordance with the laws of the Republic of Uganda.

4. DELIVERY TIMELINE

Upon acceptance of this Quotation Estimate, the Dietitian services shall commence within fourteen (14) calendar days. The full program duration is six (6) months, with monthly progress reviews conducted by the lead Dietitian. All group workshops in Uganda Kampala community centres will be scheduled in coordination with the Kampala Regional Health Authority's calendar.

5. ACCEPTANCE AND SIGNATURES

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions stated herein. This signature does not constitute a final contract but serves as an expression of intent to proceed with the Dietitian services as described.
