This Quotation Estimate is issued by GreenLife Nutrition & Dietetics Center to provide a comprehensive and transparent breakdown of costs for professional Dietitian services to be delivered in Vietnam Ho Chi Minh City. The purpose of this document is to outline the scope of work, associated fees, payment schedules, and terms governing the engagement of our qualified Dietitian team for your organization's corporate wellness program. All pricing in this Quotation Estimate is quoted in Vietnamese Dong (VND) and is subject to the standard 10% Value Added Tax (VAT) applicable in Vietnam Ho Chi Minh City.
The following services will be provided by our licensed and certified Dietitian professionals operating out of our facility in Vietnam Ho Chi Minh City. Each service is designed to meet the specific nutritional needs of your workforce and to comply with the health and safety regulations enforced by the Ho Chi Minh City Department of Health.

All Dietitian professionals assigned to this engagement hold a minimum of a Bachelor's degree in Nutrition and Dietetics from an accredited institution in Vietnam or internationally recognized universities. Each Dietitian is registered with the Vietnam Association of Nutrition and Dietetics (VAND) and maintains current professional certification. Our lead Dietitian, Dr. Nguyen Thi Lan, holds a Master's degree in Clinical Nutrition from the University of Science, Vietnam National University Ho Chi Minh City, and possesses over twelve years of experience providing dietary counseling in the Vietnam Ho Chi Minh City healthcare and corporate wellness sectors. All Dietitian staff are fluent in Vietnamese and English to ensure seamless communication with your international workforce.
As outlined in this Quotation Estimate, payment shall be structured as follows:
4.1 A 30% advance payment (VND 89,100,000) is due within seven (7) business days of the formal acceptance of this Quotation Estimate.
4.2 A 40% interim payment (VND 118,800,000) is due upon completion of the initial assessment phase and the first two group workshops.
4.3 The remaining 30% balance (VND 89,100,000) is due within fourteen (14) business days following the final delivery of all Dietitian services and submission of the comprehensive program report.
All payments shall be made via bank transfer to our account at Vietcombank, Branch: District 1, Vietnam Ho Chi Minh City. Account Name: GreenLife Nutrition & Dietetics Center. Account No: 0091018877665544.
5.1 This Quotation Estimate is valid for thirty (30) days from the date of issue. After the expiration date, all prices and service availability are subject to revision.
5.2 The Dietitian services described herein are to be performed at the client's premises in Vietnam Ho Chi Minh City or at our center in District 1, as mutually agreed upon in the final service contract.
5.3 Any additional Dietitian sessions, extended consultations, or scope changes beyond what is specified in this Quotation Estimate will be billed separately at the rates indicated in the itemized table above.
5.4 GreenLife Nutrition & Dietetics Center reserves the right to reschedule Dietitian appointments with a minimum of 48 hours' prior written notice in the event of unforeseen circumstances.
5.5 All nutritional data, health records, and personal information collected during Dietitian consultations are protected under Vietnam's Personal Data Protection Law (Law No. 13/2023/QH15) and shall not be disclosed to any third party without written consent.
5.6 This Quotation Estimate does not constitute a binding contract. A formal Service Agreement must be executed by both parties prior to the commencement of any Dietitian services.
5.7 In the event of a dispute arising from this Quotation Estimate or the subsequent service delivery, both parties agree to resolve matters amicably. Should resolution fail, disputes shall be submitted to the People's Court of District 1, Vietnam Ho Chi Minh City for adjudication.
Note: This Quotation Estimate assumes standard operating hours (Monday to Friday, 8:00 AM to 5:00 PM) in Vietnam Ho Chi Minh City. After-hours or weekend Dietitian consultations will incur a 50% surcharge on the applicable unit price. All dietary recommendations provided by our Dietitian team are for general wellness purposes and do not replace medical treatment for diagnosed conditions.By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined herein for the engagement of Dietitian services in Vietnam Ho Chi Minh City. This signature does not constitute a final contract but serves as a formal expression of intent to proceed with the services described.
