This Quotation Estimate is formally issued by Dr. Efua Mensah, a licensed Doctor General Practitioner registered with the Medical and Dental Council of Ghana, in response to the request from St. Mary's Community Health Centre for comprehensive primary healthcare services to be delivered in Ghana Accra. The Quotation Estimate has been prepared in accordance with the fee schedules established by the Ghana Health Service and the Medical and Dental Council of Ghana, ensuring full compliance with national healthcare regulations and professional standards applicable to all Doctor General Practitioner engagements in Ghana Accra.
The Doctor General Practitioner will provide the following services as part of this engagement in Ghana Accra:
- General outpatient consultations and clinical assessments for adults and paediatric patients
- Preventive healthcare screenings, including blood pressure monitoring, diabetes screening, and cardiovascular risk assessments
- Management of chronic conditions such as hypertension, type 2 diabetes, asthma, and chronic respiratory diseases
- Community health education sessions and wellness workshops in local communities across Accra
- Referral coordination to specialist physicians and tertiary hospitals within the Ghana Accra healthcare network
- Issuance of medical certificates, fitness-for-work assessments, and travel health documentation
- Quarterly health audits and reporting to the Centre Director

4.1 Payment shall be made in Ghana Cedis (GHS) via bank transfer to the account designated by the Doctor General Practitioner. The account details are: Bank of Ghana, Account Name: Dr. Efua Mensah, Account No. 0045-8821-3376, Branch: Accra Central.
4.2 A 30% advance payment is required upon acceptance of this Quotation Estimate to confirm the engagement. The remaining balance shall be invoiced monthly in arrears, with payment due within fourteen (14) calendar days of invoice issuance.
4.3 Late payments shall attract a penalty of 2% per month on the outstanding balance, in accordance with standard commercial practice in Ghana Accra.
4.4 All costs are inclusive of the Doctor General Practitioner's professional fees, clinical time, and administrative support. Any laboratory tests, imaging, or specialist referrals not listed in this Quotation Estimate shall be billed separately at prevailing Ghana Health Service rates.
5.1 This Quotation Estimate is valid for thirty (30) days from the date of issue. Upon expiry, all rates and availability must be reconfirmed in writing.
5.2 The Doctor General Practitioner commits to a minimum of four (4) full working days per week at the designated clinic in Ghana Accra, with additional on-call availability as specified in Item 07 above.
5.3 Either party may terminate this engagement with thirty (30) days' written notice. In the event of early termination, all services rendered up to the termination date shall be invoiced in full.
5.4 The Doctor General Practitioner shall maintain full professional indemnity insurance and shall comply with all data protection requirements under the Data Protection Act, 2012 (Act 843) of Ghana.
5.5 All patient records and clinical data generated during this engagement shall remain the property of St. Mary's Community Health Centre and shall be handled in strict confidence.
Note: This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties. The Doctor General Practitioner reserves the right to adjust service availability based on public health emergencies or directives issued by the Ministry of Health, Ghana.By signing below, both parties acknowledge and agree to the terms, conditions, and cost structure outlined in this Quotation Estimate for the Doctor General Practitioner services to be provided in Ghana Accra.
