This Quotation Estimate is issued by Al-Riyadh Comprehensive Medical Center to provide Al-Nakheel Corporate Health Services with a detailed and transparent financial breakdown for the engagement of a Doctor General Practitioner to deliver on-site and telemedicine primary care services within the Saudi Arabia Riyadh metropolitan area. This document outlines the scope of services, associated costs, payment terms, and conditions governing the proposed medical arrangement. All pricing is quoted in Saudi Riyals (SAR) and is subject to the applicable Value Added Tax (VAT) of 15% as mandated by the Zakat, Tax and Customs Authority (ZATCA) of Saudi Arabia.

The proposed Doctor General Practitioner engagement encompasses the following comprehensive primary care services, all to be delivered in accordance with the Saudi Ministry of Health (MOH) clinical guidelines and the Saudi Council of Health Specialties (SCFHS) standards of practice:

Comprehensive health assessments and annual physical examinations for up to 500 corporate employees located in the Riyadh business district.
Diagnosis, management, and follow-up of acute and chronic conditions including hypertension, diabetes mellitus, respiratory infections, and musculoskeletal disorders.
Preventive health screenings, immunizations, and health education sessions conducted at the client's premises in Riyadh.
Telemedicine consultations via the MOH-approved platform for non-urgent follow-ups and minor ailments.
Referral coordination to specialist departments within the Riyadh healthcare network when a patient's condition exceeds the scope of general practice.
Maintenance of electronic medical records compliant with the Saudi Health Information Exchange (HIE) standards.
Monthly reporting to the client's HR and occupational health team summarizing workforce health metrics.

4.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be subject to revision based on prevailing market conditions in Saudi Arabia Riyadh.

4.2 Payment Terms: Payment shall be made on a monthly basis within fifteen (15) days of invoice issuance. A 2% late payment penalty shall apply to overdue balances in accordance with Saudi commercial law.

4.3 Service Delivery: The Doctor General Practitioner shall be available on-site at the client's Riyadh premises from Sunday to Thursday, 08:00 to 16:00 (Saudi Standard Time), with telemedicine access available 24/7.

4.4 Regulatory Compliance: All services shall be delivered in full compliance with the Saudi Ministry of Health regulations, the Saudi Food and Drug Authority (SFDA) guidelines, and the data protection provisions of the Saudi Personal Data Protection Law (PDPL).

4.5 Cancellation & Termination: Either party may terminate this agreement with sixty (60) days' written notice. In the event of early termination, the client shall be liable for all services rendered up to the termination date.

4.6 Confidentiality: All patient medical records and corporate health data shall be treated as strictly confidential. No information shall be disclosed to third parties without written consent, in compliance with Saudi healthcare privacy regulations.

4.7 Dispute Resolution: Any disputes arising from this Quotation Estimate and the subsequent service agreement shall be resolved through the Riyadh Chamber of Commerce and Industry mediation process, and failing that, through the competent courts of Riyadh, Saudi Arabia.

Note: This Quotation Estimate does not constitute a binding contract. A formal Service Level Agreement (SLA) and Master Service Agreement (MSA) shall be executed by both parties prior to the commencement of services. The Doctor General Practitioner assigned to this engagement holds a valid SCFHS license and a current MOH practice permit for the Riyadh region.

By signing below, the client acknowledges receipt of this Quotation Estimate for the Doctor General Practitioner services in Saudi Arabia Riyadh and agrees to the terms, conditions, and pricing outlined herein. This acceptance shall serve as the basis for the execution of the definitive service contract.
