This Quotation Estimate outlines the comprehensive professional Midwife services to be rendered in Brazil São Paulo. The Midwife, a licensed and registered obstetric nurse (Enfermeira Obstetra) in accordance with the regulations of the Conselho Federal de Enfermagem (COFEN) and the Conselho Regional de Enfermagem de São Paulo (COREN-SP), will provide full-cycle prenatal, intrapartum, and postnatal care. All services are delivered in compliance with the Brazilian Ministry of Health (Ministério da Saúde) guidelines and the National Humanized Birth Policy (PNHR — Política Nacional de Humanização do Parto).
The Midwife will conduct all consultations at the client's residence or at a designated clinic in the São Paulo metropolitan area. Travel within the city limits of São Paulo is included in the base fee. Travel to the ABC Paulista region or surrounding municipalities will be subject to an additional transportation surcharge as detailed in Section 5.

- Travel beyond the city limits of São Paulo (e.g., Campinas, Santos, Sorocaba) will incur a surcharge of R$ 5,00 per kilometer, calculated one-way from the provider's base address.
- Emergency call-outs outside of standard business hours (22:00 to 06:00) will carry a 50% surcharge on the applicable service fee.
- If the Midwife is required to attend a cesarean section or any surgical intervention, the intrapartum fee will be adjusted to R$ 2.000,00, as the scope of Midwife services in Brazil does not include surgical procedures.
- All prices are quoted in Brazilian Reais (BRL) and are subject to adjustment in the event of significant changes in the Brazilian Consumer Price Index (IPCA) between the date of this Quotation Estimate and the date of service delivery.
Payment Schedule:
• 30% deposit (R$ 2.880,00) due upon acceptance of this Quotation Estimate.
• 40% (R$ 3.840,00) due at the 28th week of gestation.
• 30% (R$ 2.880,00) due within 7 days following the postnatal period.
Accepted Payment Methods: Bank transfer (TED/PIX), credit card (up to 3 installments), or cash. PIX transfers are strongly encouraged for immediate confirmation.
Late Payment: A 2% penalty plus interest of 1% per month (proportional) will apply to overdue balances, in accordance with Brazilian civil law (Código Civil, Art. 412).
- This Quotation Estimate is valid for 30 calendar days from the date of issue. After this period, all prices and availability must be reconfirmed in writing.
- The Midwife reserves the right to refer the client to an obstetrician or gynecologist if any medical complication or high-risk condition is identified during any consultation. Referral does not constitute a termination of the Midwife engagement.
- All services are governed by the laws of the Federative Republic of Brazil, specifically the State of São Paulo. Any disputes arising from this Quotation Estimate shall be subject to the jurisdiction of the courts of São Paulo, SP, Brazil.
- The Midwife maintains professional liability insurance (Seguro de Responsabilidade Profissional) with a coverage of R$ 500.000,00, valid throughout the duration of the engagement.
- Client data and medical records are protected under the Brazilian General Data Protection Law (LGPD — Lei Geral de Proteção de Dados, Lei nº 13.709/2018). No information will be shared without explicit written consent.
- Cancellation by the client after the initial prenatal consultation will result in forfeiture of the deposit. Cancellation by the provider will result in a full refund of all amounts paid.
