Please be advised that a significant portion of the Midwife services listed in this Quotation Estimate may be partially or fully covered by the German statutory health insurance (Gesetzliche Krankenversicherung) or private health insurance (Private Krankenversicherung). The client is encouraged to submit the itemized breakdown provided in this document to their respective insurance provider in Germany Berlin for pre-authorization. Our office will supply any additional documentation required by the insurer. The Midwife fees not covered by insurance shall be settled directly by the client according to the payment schedule outlined below.
4.1 This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiry date, prices may be subject to revision due to changes in the Berlin Midwife Care fee schedule or regulatory adjustments in Germany Berlin.
4.2 All Midwife services will be performed by a state-registered Midwife (Hebamme) holding a valid license issued by the Berlin Health Department (Gesundheitsamt Berlin). The specific Midwife assigned to the client will be confirmed in writing no later than fourteen (14) days prior to the first scheduled appointment.
4.3 Cancellation of any scheduled Midwife appointment in Germany Berlin must be communicated at least 48 hours in advance. Late cancellations or no-shows will be charged at 50% of the session fee.
4.4 In the event of a medical emergency during a Midwife-led home birth in Germany Berlin, the Midwife will immediately coordinate with the nearest hospital (e.g., Charité Berlin, Vivantes Klinikum Neukölln) for transfer. Any additional hospital costs are not included in this Quotation Estimate.
4.5 Payment is due within fourteen (14) days of invoice issuance. A 10% deposit is required to confirm the booking of the Midwife for the full care package. The remaining balance is payable in two installments: 50% at the 28th week of pregnancy and 50% within 14 days postpartum.
4.6 This Quotation Estimate does not constitute a binding contract. A formal service agreement (Dienstleistungsvertrag) will be executed upon the client's written acceptance of this estimate. Both parties must sign the agreement before any Midwife services commence.
4.7 All personal health data processed in connection with this Quotation Estimate and the subsequent Midwife care will be handled in strict accordance with the European General Data Protection Regulation (GDPR) and the German Federal Data Protection Act (Bundesdatenschutzgesetz, BDSG).
By signing below, the client acknowledges receipt of this Quotation Estimate for Midwife services in Germany Berlin and agrees to the terms and conditions stated herein. The service provider confirms that all listed Midwife professionals are duly licensed and insured to practice in the state of Berlin, Germany.
