This Quotation Estimate is issued by KL Midwifery & Maternal Care Centre Sdn. Bhd. to provide a comprehensive and transparent breakdown of all professional Midwife services to be rendered in Malaysia Kuala Lumpur. The purpose of this document is to outline the scope of care, associated fees, and terms governing the engagement of our registered Midwife team for the full duration of antenatal, intrapartum, and postnatal care. All pricing in this Quotation Estimate is quoted in Malaysian Ringgit (MYR) and is subject to the conditions stated herein.

2. Scope of Midwife Services – Malaysia Kuala Lumpur

Our Midwife team, comprising registered midwives with the Malaysian Nursing Board (MNB) and experienced in delivering care across the Malaysia Kuala Lumpur metropolitan area, will provide the following services as detailed in this Quotation Estimate:

Antenatal Care (ANC): A minimum of eight (8) scheduled antenatal consultations conducted at our clinic in Kuala Lumpur or via home visits within the Kuala Lumpur city limits. Each session includes vital sign monitoring, fundal height measurement, fetal heart rate auscultation, urine analysis, and nutritional counselling.
Intrapartum Care: Continuous one-to-one Midwife attendance during labour and delivery at the designated birthing facility in Malaysia Kuala Lumpur, including pain management support, positioning assistance, and immediate newborn assessment.
Postnatal Care: Four (4) postnatal home visits within the first six weeks after delivery, covering maternal recovery checks, breastfeeding support, newborn jaundice screening, and immunisation scheduling in accordance with the Malaysian Ministry of Health guidelines.
Emergency On-Call Support: Twenty-four (24) hour on-call availability of our Midwife team throughout the expected delivery window, covering all areas within Malaysia Kuala Lumpur and its immediate suburbs.
Documentation & Reporting: Preparation of a comprehensive maternity care record, birth summary, and postnatal progress report, all formatted in compliance with Malaysian healthcare documentation standards.

Note: This Quotation Estimate does not include hospital admission fees, surgical intervention costs (e.g., caesarean section), laboratory investigations beyond those listed, or pharmaceutical charges. Should any additional Midwife services be required outside the scope defined herein, a supplementary Quotation Estimate will be issued prior to the provision of such services. 4. Payment Terms

Payment for the services outlined in this Quotation Estimate shall be structured as follows:

Deposit (30%): RM 2,712.54 payable upon acceptance of this Quotation Estimate to secure the Midwife booking.
Second Instalment (40%): RM 3,616.72 payable at the 36th week of gestation.
Final Balance (30%): RM 2,712.54 payable within fourteen (14) calendar days following the completion of all postnatal visits.

Payment may be made via bank transfer to the account details below, or by FPX online banking. All transactions are processed in Malaysian Ringgit (MYR).

5. Terms & Conditions

This Quotation Estimate is valid for thirty (30) days from the date of issue. Beyond this period, pricing may be revised to reflect current market rates in Malaysia Kuala Lumpur.
All Midwife services are provided by practitioners registered with the Malaysian Nursing Board and holding valid practising certificates. Clients are entitled to request the credentials of the assigned Midwife at any time.
The Midwife team operates in strict compliance with the Malaysian Private Healthcare Facilities and Services Act 1998 (Act 586) and all applicable regulations of the Ministry of Health Malaysia.
Home visit services are limited to addresses within the Malaysia Kuala Lumpur municipal boundary (Kuala Lumpur City Hall jurisdiction). Visits to areas outside this boundary (e.g., Selangor, Putrajaya) will incur an additional travel surcharge of RM 50.00 per kilometre beyond the 15 km radius.
In the event of a medical emergency requiring transfer to a hospital, the Midwife will coordinate with the nearest public or private facility in Malaysia Kuala Lumpur. Hospital fees and specialist charges are the sole responsibility of the client and are excluded from this Quotation Estimate.
Cancellation by the client after the deposit has been paid will result in the forfeiture of 50% of the deposit. Cancellation after the 32nd week of gestation will result in the forfeiture of the full deposit.
All personal health information collected during the provision of Midwife services is protected under the Personal Data Protection Act 2010 (PDPA) of Malaysia. Data will not be shared with third parties without the client's written consent.
This Quotation Estimate constitutes the entire agreement between the parties regarding the scope and cost of the Midwife services described herein. Any amendments must be made in writing and signed by both parties.
Governing law: The laws of Malaysia. Any disputes arising from this Quotation Estimate shall be subject to the exclusive jurisdiction of the courts in Malaysia Kuala Lumpur.

6. Acceptance

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined for the Midwife services to be provided in Malaysia Kuala Lumpur.
