Important Notes for Midwife Services in Manila, Philippines

• This Quotation Estimate is valid for thirty (30) days from the date of issuance. After the expiration date, pricing for Midwife services may be subject to revision based on prevailing market rates in the Philippines.
• All Midwife services listed in this Quotation Estimate are to be performed by a Registered Midwife (RM) licensed by the Professional Regulation Commission (PRC) of the Philippines and in good standing with the Philippine Midwifery Board.
• The client agrees to a 50% down payment upon acceptance of this Quotation Estimate to secure the Midwife's schedule. The remaining 50% shall be settled within seven (7) days following the completion of all services.
• Home birth services, if selected, are subject to a preliminary home assessment by the assigned Midwife. The Midwife reserves the right to recommend hospital delivery if clinical indicators warrant, at no additional cost to the client.
• Travel within Metro Manila (Quezon City, Makati, Mandaluyong, Pasig, Taguig, and surrounding areas) is included in the Quotation Estimate. Travel beyond Metro Manila will be billed separately at PHP 50 per kilometer.
• Cancellation or rescheduling of Midwife appointments must be made at least forty-eight (48) hours in advance. Late cancellations will incur a 25% fee of the scheduled session rate.
• This Quotation Estimate does not include hospital admission fees, surgical procedures, laboratory tests ordered by a physician, or pharmaceutical costs. These will be itemized separately by the attending hospital in Manila.
• All Midwife consultations and services are conducted in compliance with the Philippine Data Privacy Act of 2012 (RA 10173). Client medical records are kept confidential and stored securely.
• In the event of a medical emergency requiring transfer to a tertiary hospital in Manila, the Midwife will coordinate with the attending obstetrician and the client's family. Additional emergency transport fees, if applicable, will be discussed prior to transfer.
• This Quotation Estimate is non-transferable and is issued specifically for the client named above. Any change in the client's address within the Philippines Manila area must be communicated to the Midwife at least 48 hours before the scheduled visit.
