1. This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, pricing may be revised to reflect changes in the Zimbabwean Dollar exchange rate, fuel costs, or updated Zimbabwe Midwives Association fee schedules.
2. The Midwife services described in this Quotation Estimate are to be performed by a registered and licensed Midwife in Zimbabwe Harare, holding a valid practising certificate issued by the Zimbabwe Midwives Association (ZMA) and the Ministry of Health and Child Care.
3. A non-refundable deposit of thirty percent (30%) of the Grand Total, amounting to ZWL 7,952.25, is required within five (5) business days of acceptance to secure the Midwife's schedule and reserve the service dates.
4. The balance of the Quotation Estimate total is due within seven (7) calendar days following the completion of all Midwife services, including the final postnatal visit.
5. In the event of a medical emergency requiring transfer to a tertiary hospital in Zimbabwe Harare (e.g., Parirenyatwa Hospital or Harare Private Hospital), the Midwife will accompany the client to the point of handover. Additional hospital fees, surgical costs, or specialist consultations are not covered by this Quotation Estimate.
6. The client agrees to provide accurate medical history and to disclose any pre-existing conditions, allergies, or medications to the Midwife prior to the commencement of services.
7. All Midwife services will be conducted in strict compliance with the Zimbabwe National Health Act, the Midwives Act, and the data protection provisions of the Zimbabwe Cyber and Data Protection Act (2021). Client records will be maintained in confidence and stored at the Zimbabwe Harare facility for a minimum of ten (10) years.
8. Cancellation by the client more than fourteen (14) days before a scheduled Midwife session will result in a 50% refund of the session fee. Cancellations within fourteen (14) days or no-shows will be charged in full.
9. This Quotation Estimate does not constitute a contract until both parties have signed and dated the acceptance section below. Upon signature, it becomes a binding agreement governed by the laws of Zimbabwe.
