This Quotation Estimate is issued by Dhaka Rehabilitation & Therapy Centre to provide a comprehensive, itemized cost breakdown for the engagement of a qualified Occupational Therapist to deliver in-home and centre-based therapy services to the client residing in Bangladesh Dhaka. This document serves as a formal financial proposal outlining the scope of services, associated fees, payment schedules, and all applicable terms governing the professional engagement of our Occupational Therapist team. The rates quoted herein reflect current market standards for rehabilitation services in the Bangladesh Dhaka metropolitan area as of June 2025.

The Occupational Therapist assigned to this engagement will hold a Bachelor of Science in Occupational Therapy (BSOT) or Master of Science in Occupational Therapy (MSOT) degree, registered with the Bangladesh Occupational Therapy Association (BOTA). The scope of services includes but is not limited to:

Comprehensive initial assessment of the patient's functional abilities, cognitive status, and daily living skills (2 sessions).
Development of an individualized treatment plan targeting upper limb mobility, fine motor coordination, and adaptive living strategies.
Weekly in-home therapy sessions conducted at the client's residence in Bangladesh Dhaka (4 sessions per week, 60 minutes each).
Centre-based group therapy sessions at our Banani facility in Bangladesh Dhaka (2 sessions per week, 90 minutes each).
Provision and fitting of assistive devices and adaptive equipment as clinically indicated.
Monthly progress evaluation and treatment plan revision by the Occupational Therapist.
Family caregiver training and home exercise program instruction (2 sessions per month).
Written progress reports submitted to the referring physician every four weeks.

The total amount of BDT 81,060.00 (Eighty-One Thousand and Sixty Taka Only) shall be payable as follows:

Advance Payment (50%): BDT 40,530.00 due upon acceptance of this Quotation Estimate and prior to the commencement of the initial assessment.
Balance Payment (50%): BDT 40,530.00 due at the conclusion of the four-week therapy program, upon submission of the final progress report by the Occupational Therapist.

Payment may be made via bank transfer to Dhaka Bank PLC, A/C No. 0045-8821-3367, or by certified cheque payable to "Dhaka Rehabilitation & Therapy Centre." All payments are to be made in Bangladeshi Taka (BDT) at the client's residence or our office in Bangladesh Dhaka.

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, rates may be subject to revision based on prevailing market conditions in Bangladesh Dhaka.
The Occupational Therapist assigned to this engagement is a licensed professional registered with BOTA. In the event of unavoidable absence, a qualified substitute of equivalent credentials will be arranged with prior notice to the client.
All in-home sessions will be conducted within the administrative boundaries of Bangladesh Dhaka (Dhaka City Corporation limits). Sessions outside this area will incur additional travel charges as per a separate addendum.
The client agrees to provide a safe, clean, and adequately lit environment for therapy sessions. The Occupational Therapist reserves the right to reschedule or cancel a session if safety concerns are identified.
Assistive devices and adaptive equipment provided under this Quotation Estimate become the property of the client upon full payment. The centre provides a six-month warranty on all fitted devices.
Confidentiality of all patient records, assessment data, and progress reports is maintained in strict accordance with the Bangladesh Health Act and professional ethical standards of the Occupational Therapist profession.
Any cancellation of a scheduled session by the client must be communicated at least 24 hours in advance. Cancellations within 24 hours will be charged at 50% of the session fee.
This Quotation Estimate does not constitute a medical diagnosis or guarantee of specific therapeutic outcomes. The Occupational Therapist will adjust the treatment plan based on the patient's clinical response.
Disputes arising from this engagement shall be resolved through amicable negotiation. If unresolved, matters shall be subject to the jurisdiction of the courts in Bangladesh Dhaka.
Extension of the therapy program beyond the initial four weeks will be quoted separately and requires written approval from both parties.

By signing below, the client acknowledges receipt and acceptance of this Quotation Estimate for Occupational Therapist services in Bangladesh Dhaka and agrees to all terms and conditions outlined herein.
