This Quotation Estimate is issued by Accra Rehabilitation & Therapy Centre to provide a comprehensive, itemised financial projection for the engagement of a qualified Occupational Therapist to deliver in-home and clinic-based therapeutic services within the Ghana Accra metropolitan area. This document outlines the scope of work, professional fees, ancillary costs, and all applicable terms governing the proposed engagement. The Occupational Therapist services described herein are designed to support clients in restoring functional independence, improving daily living skills, and facilitating safe reintegration into their home, workplace, or community environments across Ghana Accra.

The Occupational Therapist engaged under this Quotation Estimate will provide the following services to the client located in Ghana Accra:

• Initial Comprehensive Assessment: A full functional evaluation of the client's physical, cognitive, and psychosocial abilities, conducted at the client's residence or at our Osu, Accra clinic. This includes ADL (Activities of Daily Living) assessment, environmental analysis, and goal-setting consultation.
• Individualised Treatment Planning: Development of a written, evidence-based intervention plan tailored to the client's specific needs, cultural context, and living conditions within Ghana Accra.
• Therapeutic Interventions: Weekly one-on-one sessions (60 minutes each) focusing on fine motor retraining, adaptive equipment training, cognitive rehabilitation, sensory integration, and community reintegration strategies.
• Home and Workplace Modifications: On-site assessment of the client's living or working environment in Ghana Accra with written recommendations for ergonomic adjustments, accessibility improvements, and assistive device procurement.
• Family and Caregiver Training: Two dedicated sessions to educate family members or designated caregivers on therapeutic techniques, safe transfer methods, and ongoing home-based exercises.
• Progress Review and Discharge Planning: Monthly progress evaluations and a final discharge report with long-term maintenance recommendations.

• A 40% advance payment of GHS 12,075.00 is due upon acceptance of this Quotation Estimate to confirm the Occupational Therapist's schedule.
• The remaining 60% balance of GHS 18,112.50 shall be payable in two equal instalments: 50% at the midpoint of the treatment programme and 50% upon completion and delivery of the final discharge report.
• All payments are to be made via bank transfer to the account of Accra Rehabilitation & Therapy Centre, Ecobank Ghana, Accra Branch. Cheques payable to "Accra Rehabilitation & Therapy Centre Ltd."
• Late payments exceeding 14 days will incur a 2% monthly interest charge as per Ghanaian commercial practice.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all rates are subject to revision based on prevailing market conditions in Ghana Accra.
• The Occupational Therapist named in this estimate is Dr. Ama Serwaa Boateng, a registered member of the Occupational Therapy Board of Ghana (OTBG) and the Ghana Health Service. In the event of unavoidable absence, a substitute therapist of equivalent qualification will be assigned with prior client consent.
• All services will be delivered in compliance with the Occupational Therapy Act, 2019 (Act 1001) of Ghana and the professional code of ethics of the Occupational Therapy Board of Ghana.
• Client confidentiality is strictly maintained in accordance with the Data Protection Act, 2012 (Act 843) of Ghana. All clinical records will be stored securely and accessible only to authorised personnel.
• This Quotation Estimate does not constitute a binding contract until both parties have signed and returned the acceptance section below. Upon signature, a formal Service Agreement will be executed.
• Any changes to the scope of services, additional sessions, or modifications to the treatment plan will be documented in a supplementary Quotation Estimate addendum, subject to mutual written agreement.
• The estimated costs for assistive devices (Item 7) are provisional and based on current supplier quotations in Ghana Accra. Final pricing will be confirmed in writing prior to procurement.
• Disputes arising from this engagement shall be resolved through mediation in accordance with the laws of the Republic of Ghana, with jurisdiction in the courts of Accra.
