This Quotation Estimate document has been prepared by RehabCare Occupational Therapy Centre, Bangalore, to provide a comprehensive and transparent financial breakdown for the professional services of a qualified Occupational Therapist to be rendered at the client's residence or at our facility in Bangalore, India. This estimate covers all anticipated costs associated with the therapeutic intervention programme, including assessment, treatment sessions, adaptive equipment recommendations, and follow-up consultations. All pricing is quoted in Indian Rupees (INR) and is subject to the terms and conditions outlined in this document.
The Occupational Therapist engaged under this Quotation Estimate will provide evidence-based therapeutic interventions tailored to the client's specific functional needs. The scope of services includes but is not limited to: comprehensive functional assessment, fine and gross motor skill development, activities of daily living (ADL) training, cognitive rehabilitation, sensory integration therapy, adaptive strategy training, and home environment modification guidance. All services will be delivered in accordance with the standards set by the Rehabilitation Council of India (RCI) and the guidelines of the World Federation of Occupational Therapists (WFOT).

- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, the rates may be revised based on prevailing market conditions in Bangalore, India.
- A 50% advance payment of the total Quotation Estimate amount is required to confirm the engagement of the Occupational Therapist and to schedule the initial assessment.
- The remaining 50% balance shall be payable in two equal instalments: 25% at the mid-programme review and 25% upon completion of the final session and submission of the discharge summary.
- All payments shall be made via bank transfer, UPI, or demand draft to the account details provided by RehabCare Occupational Therapy Centre, Bangalore. Cash payments exceeding ₹10,000 are not accepted in compliance with Indian financial regulations.
- The Occupational Therapist reserves the right to modify the treatment plan based on the client's progress, clinical response, and evolving functional needs. Any additional sessions beyond the scope outlined in this estimate will be billed separately at the per-session rates indicated above.
- Cancellations of scheduled sessions must be communicated at least 24 hours in advance. Sessions cancelled with less than 24 hours' notice will be charged at 50% of the session fee.
- All clinical records, assessment reports, and progress documentation generated during the course of therapy are the property of the client and will be shared upon request. The Occupational Therapist is bound by the confidentiality provisions of the Indian Medical Council (Professional Conduct) Regulations.
- This Quotation Estimate does not include the cost of adaptive equipment, orthotic devices, or assistive technology that may be recommended. A separate itemized estimate for such items will be provided if required.
- Any disputes arising from this Quotation Estimate shall be subject to the jurisdiction of the competent courts in Bangalore, Karnataka, India.
- This document constitutes a good-faith estimate and is not a binding contract until both parties have signed and the advance payment has been received.
