This Quotation Estimate is issued by Osaka Rehabilitation & Therapy Services Co., Ltd. to formally present the projected costs, scope of work, and professional terms associated with the provision of a qualified Occupational Therapist to the Nishinomiya Care & Wellness Group facility located in Japan Osaka. This document serves as a binding financial proposal upon mutual acceptance and signature by both parties. All figures presented herein are denominated in Japanese Yen (JPY) and are subject to the current 10% Consumption Tax applicable in Japan Osaka unless otherwise stated.

The Occupational Therapist engaged under this Quotation Estimate shall be a licensed professional registered with the Japanese Occupational Therapist Association (JOTA) and shall hold a valid national qualification certificate issued by the Ministry of Health, Labour and Welfare of Japan. The Occupational Therapist will be assigned on a full-time basis (40 hours per week, Monday through Friday) to deliver comprehensive therapeutic interventions to residents and patients at the client's facility in Japan Osaka.

2.1 Core Responsibilities of the Occupational Therapist

  • Conduct individualized and group-based occupational therapy sessions for patients recovering from stroke, orthopedic surgery, neurological conditions, and age-related functional decline.
  • Design, implement, and monitor personalized treatment plans in coordination with the facility's medical director and nursing staff.
  • Perform functional assessments, activity of daily living (ADL) evaluations, and environmental adaptations for the facility in Japan Osaka.
  • Provide therapeutic education to caregivers, family members, and support staff regarding home-based rehabilitation strategies.
  • Maintain detailed clinical documentation in compliance with Japanese medical record-keeping regulations and the facility's internal quality assurance protocols.
  • Participate in weekly multidisciplinary team meetings and monthly case-review conferences.
  • Supervise and mentor junior therapy assistants and student Occupational Therapists on placement from Osaka University or Kansai University of Health Sciences.

Payment for the services outlined in this Quotation Estimate shall be made on a monthly basis. Invoices will be issued on the 25th of each month for services rendered in the preceding calendar month. Payment is due within 30 days of the invoice date via bank transfer to the account designated by Osaka Rehabilitation & Therapy Services Co., Ltd. Late payments shall incur a penalty interest rate of 1.5% per month, in accordance with the Japanese Late Payment of Compensation Act. The one-time equipment cost (Item 3) shall be invoiced separately upon delivery and acceptance at the facility in Japan Osaka.

  1. This Quotation Estimate is valid for 30 calendar days from the date of issue. Beyond this period, pricing may be revised to reflect changes in the Japanese labor market, statutory wage adjustments, or supply costs in Japan Osaka.
  2. The Occupational Therapist shall comply with all applicable Japanese labor laws, the Act on the Practice of Physical Therapists and Occupational Therapists, and the facility's internal policies and code of conduct.
  3. Either party may terminate this engagement with 60 days' written notice. In the event of early termination, a pro-rata adjustment of the remaining monthly fees will be applied.
  4. Osaka Rehabilitation & Therapy Services Co., Ltd. reserves the right to replace the assigned Occupational Therapist with a qualified substitute of equivalent or higher credentials, provided the client is given at least 14 days' prior written notice.
  5. All clinical data, patient records, and proprietary information generated during the engagement shall remain the confidential property of the client facility in Japan Osaka and shall be handled in strict accordance with the Japanese Act on the Protection of Personal Information (APPI).
  6. This Quotation Estimate does not constitute a guarantee of specific clinical outcomes. The Occupational Therapist will exercise professional judgment in line with evidence-based practice standards recognized in Japan.
  7. Any disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. If unresolved, matters shall be submitted to the Osaka District Court for adjudication under the laws of Japan.
Note: This Quotation Estimate assumes standard operating hours (08:30–17:30, Monday to Friday) at the client's facility in Japan Osaka. Overtime, weekend, or public-holiday sessions (e.g., Golden Week, Obon, New Year) will be billed at 1.5× the standard hourly rate and require prior written approval from the client's Director of Operations.

By signing below, both parties acknowledge that they have read, understood, and agree to all terms, conditions, and cost figures set forth in this Quotation Estimate for the engagement of an Occupational Therapist in Japan Osaka.
