1. Purpose of This Quotation EstimateThis Quotation Estimate has been prepared by Auckland Allied Health Services Ltd to provide Mr. Jonathan T. Whitfield with a comprehensive and transparent breakdown of the professional services to be delivered by our registered Occupational Therapist within the New Zealand Auckland metropolitan region. This document outlines the scope of assessment, intervention, and follow-up services, along with the associated costs, payment schedules, and terms governing the engagement. All pricing reflects current New Zealand market rates for allied health services as of June 2025 and is compliant with the Health and Disability Commissioner's Code of Health and Disability Services Consumers' Rights.
2. Scope of Occupational Therapist ServicesThe Occupational Therapist assigned to this engagement, Ms. Sarah K. McAllister (NZOT Registration No. OT-2019-44782), will provide the following services tailored to the client's functional needs within the New Zealand Auckland community setting:
- Comprehensive Initial Assessment (2.5 hours): A full functional assessment conducted at the client's residence in Parnell, Auckland, evaluating upper limb dexterity, fine motor coordination, cognitive processing, sensory integration, and daily living activities. This includes a home environment audit for accessibility and safety modifications.
- Individualised Treatment Plan Development (1 hour): Preparation of a written intervention plan aligned with the client's personal goals, incorporating evidence-based therapeutic strategies and community resource referrals specific to the New Zealand Auckland region.

This Quotation Estimate is structured to minimise financial burden on the client. Payment shall be made as follows:
- Deposit: 20% of the total amount ($713.00) is due upon acceptance of this Quotation Estimate to secure the Occupational Therapist's schedule.
- Progress Payments: 50% of the remaining balance ($1,426.00) is due upon completion of Session 5 (mid-point of the intervention programme).
- Final Payment: The remaining balance ($1,426.00) is due within 14 calendar days of the final Progress Review and Discharge Summary session.
- Payment Methods: Bank transfer to Auckland Allied Health Services Ltd (BNZ, Account 01-0312-0456789-000), EFTPOS, or credit/debit card (Visa, Mastercard, Amex). A 2% surcharge applies to card payments.
- ACC / Insurance: Should the client hold an active ACC claim or private health insurance policy covering Occupational Therapy in New Zealand, our billing team will assist with direct claims processing. Any co-payment or excess will be the client's responsibility.
- This Quotation Estimate is valid for 30 days from the date of issue. After this period, rates may be subject to revision based on current New Zealand Auckland market conditions and provider fee schedules.
- All services will be delivered by a registered Occupational Therapist holding current membership with the New Zealand Association of Occupational Therapists (NZAOTh) and a valid Practising Certificate issued by the Health and Disability Commissioner.
- The client agrees to provide a safe and accessible environment for in-home sessions. The Occupational Therapist reserves the right to decline service in environments posing a risk to health and safety.
- Cancellations must be provided with a minimum of 48 hours' notice. Late cancellations or no-shows will incur a fee of 50% of the session rate.
- All clinical records, assessment reports, and treatment plans are the property of Auckland Allied Health Services Ltd and are stored in accordance with the New Zealand Privacy Act 2020. The client retains the right to access their records upon written request.
- This engagement is governed by the laws of New Zealand. Any disputes arising from this Quotation Estimate shall be resolved through the Dispute Resolution Service of the New Zealand Disputes Tribunal, Auckland District.
- The Occupational Therapist may refer the client to other allied health professionals, medical practitioners, or community services within the New Zealand Auckland region as clinically indicated, with the client's prior written consent.
- Force majeure events, including but not limited to severe weather events, public health emergencies, or government-mandated restrictions affecting service delivery in Auckland, may result in rescheduling of sessions without penalty to either party.
By signing below, the client acknowledges that they have read, understood, and agree to all terms, conditions, and pricing outlined in this Quotation Estimate. The Occupational Therapist services described herein will commence on or after the date specified, subject to availability and confirmation of the initial deposit.
