This Quotation Estimate is issued by Lagos Allied Health & Rehabilitation Centre in response to the request for professional Occupational Therapist services to be delivered within the Nigeria Lagos metropolitan area. Our centre has been providing evidence-based occupational therapy interventions to residents of Lagos and the wider Lagos State for over fourteen years. The services outlined in this Quotation Estimate are designed to address the functional rehabilitation needs of the client as assessed during the initial consultation conducted on 28 May 2025 at our Victoria Island facility.

The Occupational Therapist assigned to this engagement, Mrs. Funke Adeyemi, MSc (OT), is a fully registered practitioner with the Allied Health Professions Council of Nigeria (AHPCN) and holds a valid practice licence for the Lagos State jurisdiction. She brings over eleven years of clinical experience in paediatric, adult, and geriatric occupational therapy, with specialisations in post-stroke rehabilitation, sensory integration, and adaptive equipment prescription.

Note: All prices in this Quotation Estimate are quoted in Nigerian Naira (NGN) and are inclusive of Value Added Tax (VAT) at the prevailing rate of 7.5% as applicable in Nigeria Lagos. Home visit charges include transportation within the Lagos metropolitan area. Travel to locations outside Lagos Mainland and Island may attract an additional surcharge of ₦5,000 per visit.

3.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Upon expiry, the Occupational Therapist service rates may be subject to revision based on prevailing economic conditions in Nigeria Lagos.

3.2 A non-refundable deposit of fifty percent (50%) of the total Quotation Estimate amount, amounting to ₦1,072,500, is required to secure the client's place in the therapy schedule. The remaining balance shall be payable in two equal monthly instalments.

3.3 The Occupational Therapist reserves the right to reschedule sessions with a minimum of twenty-four (24) hours' prior notice. Cancellations by the client with less than twelve (12) hours' notice will be charged at fifty percent of the session fee.

3.4 All clinical records, assessment reports, and progress documentation generated during the engagement remain the property of Lagos Allied Health & Rehabilitation Centre. The client is entitled to one (1) free copy of the final discharge summary.

3.5 This Quotation Estimate does not constitute a guarantee of specific clinical outcomes. The Occupational Therapist will exercise professional judgement in modifying the treatment plan as the client's condition evolves.

3.6 In the event of force majeure, including but not limited to power outages, flooding, or public health emergencies affecting Nigeria Lagos, sessions may be rescheduled without penalty to either party.

3.7 All disputes arising from this Quotation Estimate shall be resolved through mediation in accordance with the laws of the Federal Republic of Nigeria, with jurisdiction vested in the courts of Lagos State.

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions outlined herein. The engagement of the Occupational Therapist shall commence within five (5) working days of receipt of the initial deposit.
