This Quotation Estimate has been prepared specifically to outline the professional fees, service deliverables, and associated costs for the engagement of a qualified Occupational Therapist to provide comprehensive therapy services within the Qatar Doha metropolitan area. The Occupational Therapist services described herein are designed to meet the clinical, educational, and functional rehabilitation needs of the client as discussed during our initial consultation on 28 May 2025. All pricing in this Quotation Estimate is denominated in Qatari Riyals (QAR) and reflects current market rates for licensed Occupational Therapist professionals operating in Qatar Doha as of the date of issue.

The Occupational Therapist assigned to this engagement holds a Master of Science in Occupational Therapy (MSc OT) from an internationally accredited institution, is registered with the Qatar Ministry of Public Health, and possesses a minimum of eight (8) years of clinical experience in paediatric and adult rehabilitation. The Occupational Therapist is additionally certified in the Sensory Integration (SI) framework, the Canadian Occupational Performance Measure (COPM), and the Assessment of Children's Thinking Skills (ACTS). All clinical practice is conducted in full compliance with the regulatory standards governing allied health professionals in Qatar Doha.

All in-clinic Occupational Therapist sessions will be conducted at our fully equipped therapy facility located in the Al Sadd District, Doha, Qatar. Home assessment visits will be carried out at the client's residential address within the Qatar Doha municipality boundaries. The Occupational Therapist will utilise a dedicated clinical vehicle for all home visits. Sessions are scheduled between Monday and Friday, 08:00 AM to 06:00 PM, in accordance with the standard working week in Qatar Doha. No services will be scheduled on Fridays after 12:00 PM or on public holidays as declared by the State of Qatar.

Terms Governing This Quotation Estimate

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Upon expiry, all rates for the Occupational Therapist services listed herein are subject to revision based on prevailing market conditions in Qatar Doha.
Payment is due within fourteen (14) days of invoice issuance. A 50% advance payment is required to confirm the booking of the Occupational Therapist and secure the scheduled therapy slots.
Accepted payment methods include bank transfer to our Qatar National Bank (QNB) account, credit/debit card, and cash (receipts provided). All transactions are processed in Qatari Riyals (QAR).
Cancellations of scheduled Occupational Therapist sessions must be communicated at least 24 hours in advance. Late cancellations or no-shows will be charged at 75% of the session fee.
This Quotation Estimate does not include the cost of assistive devices, orthotic equipment, or adaptive materials. A separate itemised estimate will be provided if such items are recommended by the Occupational Therapist during the home assessment.
All clinical data, session notes, and progress reports generated by the Occupational Therapist are the confidential property of the client and are handled in strict accordance with Qatar's data protection regulations and the facility's privacy policy.
Any additional Occupational Therapist sessions beyond the scope defined in this Quotation Estimate will be billed at the standard unit rates listed in Section 2, subject to prior written approval by the client.
This Quotation Estimate is non-transferable and is issued exclusively to the named client for use within the State of Qatar, Doha jurisdiction.
Disputes arising from this Quotation Estimate or the subsequent service agreement shall be resolved through amicable negotiation, failing which they shall be subject to the jurisdiction of the competent courts in Doha, Qatar.

By signing below, the client acknowledges receipt of this Quotation Estimate for Occupational Therapist services in Qatar Doha and agrees to the terms, conditions, and pricing outlined in this document. The service provider confirms that the Occupational Therapist named in this Quotation Estimate is available and qualified to deliver the described scope of work.
