4.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, pricing for Optometrist services and optical products may be subject to revision based on current market conditions in China Shanghai.
4.2 Payment Terms: A deposit of fifty percent (50%) of the total Quotation Estimate amount is due upon acceptance. The remaining balance shall be settled within fourteen (14) business days following the completion of all Optometrist services and lens dispensing.
4.3 Cancellation Policy: Should the client wish to cancel or modify the scope of Optometrist services after acceptance of this Quotation Estimate, a cancellation fee of twenty percent (20%) of the total estimated value shall apply. No refund will be issued for services already rendered by the Optometrist team.
4.4 Service Standards: All Optometrist procedures will be performed by licensed optometrists registered with the Shanghai Municipal Health Commission. Equipment used will be calibrated and maintained in compliance with PRC medical device regulations (NMPA certified).
4.5 Warranty: Dispensed optical lenses carry a twelve-month (12-month) manufacturer warranty against defects in materials and workmanship. Frame warranties are provided by the respective manufacturer as stated in the accompanying documentation.
4.6 Data Privacy: All patient records and Optometrist diagnostic data collected during this engagement in China Shanghai shall be handled in strict accordance with the Personal Information Protection Law of the People's Republic of China (PIPL, effective 1 November 2021).
4.7 Governing Law: This Quotation Estimate and any resulting service agreement shall be governed by and construed in accordance with the laws of the People's Republic of China. Any disputes shall be resolved through arbitration at the Shanghai Arbitration Commission.
Note: This Quotation Estimate is a good-faith estimate of costs for the Optometrist services described herein. Final invoicing may vary by up to five percent (5%) depending on the specific clinical findings of the Optometrist during examination and any additional corrective measures deemed necessary. All prices are quoted in Chinese Yuan (CNY / RMB). For international clients, equivalent payment in USD may be arranged at the PBOC reference exchange rate on the date of invoicing.By signing below, the client acknowledges and accepts this Quotation Estimate for the Optometrist services to be provided in China Shanghai. This signature constitutes a binding agreement to the terms and conditions outlined in Section 4 of this document.
