All Optometrist professionals listed in this Quotation Estimate hold valid practicing licenses issued by the Ordre des Médecins et Optométristes de la République Démocratique du Congo and are fully registered to practice in Kinshasa and the surrounding provinces of the DR Congo.

The scope of Optometrist services covered by this Quotation Estimate includes comprehensive eye examinations, refraction assessments, contact lens fitting, low-vision rehabilitation, pediatric optometry, and post-surgical follow-up care. Each Optometrist will be stationed at the designated clinic facility in the Gombe district of Kinshasa and will be available for consultation six (6) days per week, from 07:00 to 17:00, with a one-hour lunch break. The Optometrist team will also provide monthly community outreach screenings in underserved neighborhoods of Kinshasa, including Matete, Ngaliema, and Limete, as part of the public health mandate of the DR Congo.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, Clinique Optique Kinshasa reserves the right to revise the pricing of the Optometrist services listed herein due to fluctuations in the exchange rate between the US Dollar and the Congolese Franc (CDF) in the DR Congo.
• All Optometrist professionals engaged under this Quotation Estimate shall comply with the labor laws of the Democratic Republic of Congo, including the Code du Travail applicable in Kinshasa. Social security contributions (CNSS) shall be handled by the provider.
• Payment terms: 40% advance upon contract signature, 40% upon completion of the sixth (6th) month of Optometrist service delivery, and 20% upon final acceptance and completion of the twelve (12) month period. Payments shall be made in US Dollars via bank transfer to the account of Clinique Optique Kinshasa, Kinshasa, DR Congo.
• The Optometrist team shall submit a detailed monthly activity report to the Ministère de la Santé Publique by the 5th business day of each month, documenting the number of patients seen, diagnoses made, and community outreach activities conducted in Kinshasa.
• Any modification to the scope of Optometrist services outlined in this Quotation Estimate must be agreed upon in writing by both parties. Unilateral changes to the service delivery schedule or location within DR Congo are not permitted without prior written consent.
• This Quotation Estimate does not constitute a binding contract. A formal service agreement shall be executed by both parties prior to the commencement of Optometrist services in Kinshasa.
• Disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. In the event of failure to reach a settlement, the matter shall be submitted to the competent courts of Kinshasa, DR Congo, in accordance with the national legal framework.
