As a registered Optometrist clinic in Iraq Baghdad, we adhere to the standards set forth by the Iraqi Ministry of Health and the Iraqi Optometric Association. All services listed in this Quotation Estimate will be performed by a board-certified Optometrist with a minimum of ten years of clinical experience in the Baghdad region.

3. Terms and Conditions of This Quotation Estimate

Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, prices may be adjusted to reflect changes in import costs, exchange rates, or supplier pricing within Iraq Baghdad.
Payment Terms: A 50% deposit is required at the time of order confirmation. The remaining 50% balance is due upon completion of lens manufacturing and prior to final fitting by the Optometrist.
Manufacturing Timeline: Standard single-vision lenses will be ready within 5–7 business days. Progressive multifocal lenses may require 10–14 business days depending on the complexity of the prescription and availability of materials in Baghdad.
Warranty: All lenses and frames carry a 12-month manufacturer warranty against defects in materials and workmanship. This warranty does not cover damage caused by misuse, accidental breakage, or improper cleaning.
Prescription Validity: The optical prescription issued by our Optometrist is valid for one (1) year in accordance with Iraqi health regulations. A new examination is required for any subsequent order.
Refunds and Exchanges: Custom-made lenses cannot be returned or exchanged unless a manufacturing defect is confirmed by our Optometrist within 14 days of delivery.
Tax Compliance: All prices are subject to the 5% Value Added Tax (VAT) as mandated by the Iraqi General Commission for Zaka, Tax, and Customs. A tax receipt will be provided upon full payment.
Service Location: All examinations, fittings, and consultations will take place at our clinic located in Al-Mansour District, Baghdad, Iraq. Home-visit services are available at an additional charge of 25,000 IQD per visit.

Note to Client: This Quotation Estimate is a good-faith estimate of costs and is not a binding contract until both parties have signed and the deposit has been received. Al-Rafidain Vision & Eye Care Center reserves the right to modify the scope of services if the Optometrist identifies additional clinical needs during the examination that were not anticipated at the time of this Quotation Estimate. Any such modifications will be communicated to the client in writing before additional charges are incurred.

4. Acceptance and Authorization

By signing below, the client acknowledges receipt of this Quotation Estimate for Optometrist services in Iraq Baghdad and agrees to the terms and conditions outlined above. The client confirms that the information provided is accurate and authorizes Al-Rafidain Vision & Eye Care Center to proceed with the requested services upon receipt of the deposit.
