1. Purpose of This Quotation Estimate

This Quotation Estimate is issued by ClearSight Optometry Center, a licensed and accredited Optometrist practice operating in Qatar Doha, to provide Al Rayyan Corporate Health Group with a comprehensive and transparent breakdown of costs associated with annual optometric screening, diagnostic assessments, and corrective lens dispensing for their employee wellness programme. This document serves as a formal Quotation Estimate and does not constitute a binding contract until both parties have reviewed, approved, and executed the accompanying service agreement. All pricing reflected in this Quotation Estimate is specific to the Optometrist services rendered within the Qatar Doha metropolitan area and is subject to the terms outlined herein.

2. Scope of Optometrist Services

The following Optometrist services have been scoped for delivery at our Qatar Doha facility. Each service is performed by a board-certified Optometrist holding a valid licence issued by the Qatar Ministry of Public Health. The scope encompasses routine vision examinations, advanced diagnostic imaging, and the dispensing of corrective optical devices tailored to the individual needs of each employee.

3. Terms and Conditions of This Quotation Estimate

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, pricing for the Optometrist services listed herein may be subject to revision based on material costs, regulatory changes in Qatar Doha, or operational adjustments.
All Optometrist services will be delivered at the ClearSight Optometry Center facility located in Doha, Qatar, during standard operating hours of 08:00 to 20:00, Sunday through Thursday. After-hours or weekend appointments may incur an additional 15% surcharge.
Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, with the remaining 50% due within fourteen (14) days of completion of all scheduled Optometrist services. Payment shall be made in Qatari Riyal via bank transfer to the account details specified in the invoice.
This Quotation Estimate assumes a patient volume as stated in the quantity column. Should the actual number of patients exceed the estimated quantities, additional services will be billed at the same unit rates. A reduction of more than 20% in patient volume may result in a revised Quotation Estimate.
All Optometrist practitioners operating in Qatar Doha under this engagement hold valid professional licences and malpractice insurance. The client shall be provided with a copy of the lead Optometrist's credentials upon request.
Confidentiality: All patient data collected during the Optometrist examinations will be handled in strict accordance with Qatar's data protection regulations and the facility's internal privacy policy. No individual patient records will be disclosed to third parties without written consent.
This Quotation Estimate does not include emergency ophthalmic surgery, hospitalisation, or referral to a specialist ophthalmologist. Should the Optometrist identify a condition requiring surgical intervention, a separate referral and cost estimate will be provided.
Any modifications to the scope of Optometrist services must be agreed upon in writing by both parties prior to execution. Verbal amendments to this Quotation Estimate shall not be binding.

4. Notes and Additional Information

Note 1: The pricing in this Quotation Estimate reflects the standard Optometrist service rates applicable in Qatar Doha as of June 2025. These rates are competitive within the Doha healthcare market and are inclusive of all diagnostic equipment usage, consumables, and professional consultation time.

Note 2: ClearSight Optometry Center is registered with the Qatar Ministry of Public Health and operates in full compliance with all regulatory standards governing Optometrist practices in Qatar Doha. Our facility is equipped with state-of-the-art diagnostic technology, including digital retinal cameras, OCT scanners, and automated refraction systems.

5. Acceptance and Authorisation

By signing below, both parties acknowledge that they have read, understood, and agree to the terms, conditions, and pricing outlined in this Quotation Estimate for the Optometrist services to be delivered in Qatar Doha. This signature converts the Quotation Estimate into a binding service order, subject to the execution of the master service agreement.
