This Quotation Estimate is formally submitted by Kampala Vision Care & Optometry Centre to the Kampala Municipal Health Authority in response to the request for professional Optometrist services in Uganda Kampala. Our practice, duly registered and licensed by the Uganda Optometrists Association (UOA) and the National Drug Authority (NDA), provides comprehensive eye care services to the residents of Uganda Kampala and its surrounding districts.

The scope of this Quotation Estimate covers the provision of a qualified Optometrist team for a six-month community eye screening and vision correction programme targeting approximately 5,000 residents across the Kampala Capital City Metropolitan Area. All services will be delivered in accordance with the standards set by the Ministry of Health, Republic of Uganda, and the professional guidelines of the Uganda Optometrists Association.

3.1 Validity: This Quotation Estimate shall remain valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, all prices and terms are subject to revision due to fluctuations in the Ugandan Shilling exchange rate and changes in the cost of optical materials in Uganda Kampala.

3.2 Payment Terms: Payment shall be made in three equal instalments: 40% upon contract signing, 35% upon completion of the third month of service delivery, and 25% upon final programme completion and submission of the comprehensive report to the Kampala Municipal Health Authority.

3.3 Professional Standards: All Optometrist services shall be rendered by practitioners holding valid licences issued by the Uganda Optometrists Association. Each Optometrist assigned to this programme in Uganda Kampala shall carry a minimum of five (5) years of post-qualification clinical experience.

3.4 Equipment and Materials: All diagnostic instruments, optical lenses, frames, and consumables specified in this Quotation Estimate shall be of international quality standards and sourced from NDA-approved suppliers operating within Uganda Kampala.

3.5 Confidentiality: All patient records and health data collected during the programme shall be handled in strict compliance with the Uganda Data Protection and Privacy Act, 2019. No patient information shall be disclosed to third parties without written consent.

3.6 Cancellation and Termination: Either party may terminate the engagement with a written notice of fourteen (14) days. In the event of cancellation by the client, a 15% administrative fee on the unfulfilled portion of this Quotation Estimate shall apply.

3.7 Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved through mediation in accordance with the laws of the Republic of Uganda, with jurisdiction vested in the courts of Uganda Kampala.

Note: This Quotation Estimate does not include Value Added Tax (VAT) at the prevailing rate of 18%, which shall be added to the grand total upon final invoicing. All costs are quoted in Ugandan Shillings (UGX) unless otherwise stated. The Optometrist team shall operate Monday through Saturday, 08:00 to 17:00 East Africa Time (EAT), at designated sites within Uganda Kampala.

By signing below, the parties acknowledge and accept the terms, conditions, and pricing outlined in this Quotation Estimate for Optometrist services in Uganda Kampala. This document constitutes a binding agreement upon countersignature by both parties.
