Payment for the services outlined in this Quotation Estimate is due within 14 days of the date of issue. We accept payment via bank transfer, debit/credit card (Visa, Mastercard, Amex), and contactless payment at our Birmingham practice. A 50% deposit is required to secure your appointment with the Optometrist and to reserve your selected optical frame and lens combination. The remaining balance is payable at the point of dispensing. Late payments will incur a charge of 4% per month on the outstanding balance in accordance with the Late Payment of Commercial Debts (Interest) Act 1998, applicable throughout the United Kingdom.
Terms and Conditions of this Quotation Estimate
- This Quotation Estimate is valid for a period of 30 days from the date of issue. Prices are subject to change after the validity period expires.
- All Optometrist services are provided by a fully registered and GOC-licensed practitioner operating in Birmingham, United Kingdom. The Optometrist holds professional indemnity insurance in excess of £5,000,000.
- The client acknowledges that this Quotation Estimate does not constitute a medical diagnosis. Should the Optometrist identify a condition requiring referral to an ophthalmologist or GP in the Birmingham area, the client will be advised accordingly at no additional cost for the referral itself.
- Optical products (frames and lenses) are covered by a 12-month manufacturer's warranty. Any defects in materials or workmanship will be repaired or replaced at no charge within this period.
- This Quotation Estimate is prepared specifically for the named client and may not be transferred or assigned to a third party without prior written consent from Clarendon Vision Optometry Centre.
- All services are subject to the Data Protection Act 2018 and UK GDPR. Client records and optical prescriptions are stored securely in Birmingham and will not be shared with third parties without explicit consent.
- Should the client wish to cancel any portion of the services after the deposit has been paid, the deposit is non-refundable. Full cancellation prior to the deposit is permitted without penalty.
- This Quotation Estimate is governed by the laws of England and Wales. Any disputes arising from this estimate shall be subject to the exclusive jurisdiction of the courts in Birmingham, United Kingdom.
- Our practice operates Monday to Friday, 09:00–18:00, and Saturday 09:00–13:00. Appointments with the Optometrist must be booked in advance. Emergency same-day appointments may be available subject to the Optometrist's clinical schedule.
