This Quotation Estimate is issued by MedCare Pharmacist Solutions Sdn. Bhd. to provide comprehensive Pharmacist services for Grand Horizon Medical Centre located in Malaysia Kuala Lumpur. The Pharmacist engagement covers clinical pharmacy support, medication management, patient counselling, and regulatory compliance in accordance with the Pharmacy and Poisons Act 1911 and guidelines set by the Pharmacy Board of Malaysia. All services will be delivered by a registered Pharmacist holding a valid Practising Certificate issued by the Pharmacy Board of Malaysia.

- This Quotation Estimate is valid for thirty (30) days from the date of issue. Any acceptance beyond the validity period may be subject to revised pricing due to market fluctuations in Malaysia Kuala Lumpur.
- All Pharmacist services outlined in this Quotation Estimate shall be performed by a Pharmacist registered with the Pharmacy Board of Malaysia and holding a current Practising Certificate. The Pharmacist shall comply with all professional codes of conduct as stipulated by the Malaysian Pharmaceutical Society.
- Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, and the remaining 50% shall be settled in monthly instalments within fourteen (14) days from the end of each calendar month.
- All amounts stated in this Quotation Estimate are in Malaysian Ringgit (MYR) and are inclusive of all applicable taxes unless otherwise noted. Sales and Service Tax (SST) at 6% has been applied as per the Inland Revenue Board of Malaysia (LHDN) regulations.
- The Pharmacist engagement is subject to a minimum contract period of twelve (12) months. Early termination by either party requires a written notice of sixty (60) days and may incur a penalty of two (2) months' service fee.
- MedCare Pharmacist Solutions Sdn. Bhd. shall provide a replacement Pharmacist of equivalent qualifications within forty-eight (48) hours in the event of illness, leave, or resignation of the assigned Pharmacist.
- All patient data and medical records handled by the Pharmacist in Malaysia Kuala Lumpur shall be treated as strictly confidential in accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia.
- This Quotation Estimate does not constitute a binding contract until a formal Service Agreement is executed by both parties. The terms herein are indicative and subject to final negotiation.
- Any additional services beyond the scope described in this Quotation Estimate shall be quoted separately and require written approval from the client prior to commencement.
- Disputes arising from this Quotation Estimate or the subsequent Pharmacist service engagement shall be resolved through mediation in Kuala Lumpur, Malaysia, in accordance with the laws of Malaysia.
By signing below, the client acknowledges and accepts this Quotation Estimate for Pharmacist services in Malaysia Kuala Lumpur. The signed copy shall be returned to MedCare Pharmacist Solutions Sdn. Bhd. within the validity period to confirm the engagement.
