This Quotation Estimate is issued by Guangzhou Physiotherapy & Rehabilitation Centre Co., Ltd. to provide a comprehensive and itemised cost breakdown for the engagement of a qualified Physiotherapist to deliver in-house rehabilitation and physiotherapy services at the client's premises located in China Guangzhou. The scope of work outlined in this Quotation Estimate covers a twelve-month service period commencing on 1 September 2025 and concluding on 31 August 2026. All pricing, deliverables, and terms set forth herein are binding upon mutual acceptance and signature.
The contracted Physiotherapist shall be a licensed professional registered with the Guangdong Provincial Health Commission and shall hold a minimum of five (5) years of post-qualification clinical experience in orthopaedic, sports, and neurological physiotherapy. The Physiotherapist will be stationed full-time at the client's facility in China Guangzhou and will provide the following services:
- Assessment & Diagnosis: Comprehensive initial physiotherapy assessments for all referred patients, including gait analysis, range-of-motion evaluation, manual muscle testing, and functional movement screening. A minimum of forty (40) new patient assessments per month shall be conducted.
- Manual Therapy & Treatment: Hands-on physiotherapist interventions including joint mobilisation, soft-tissue massage, myofascial release, cupping therapy, and acupuncture-assisted rehabilitation sessions. A minimum of two hundred (200) treatment sessions per month are included.
- Therapeutic Exercise & Rehabilitation Programming: Design and supervision of individualised exercise programmes for post-surgical recovery, chronic pain management, sports injury rehabilitation, and elderly mobility improvement. Group exercise classes (up to 12 participants) shall be conducted three times per week.
- Electrotherapy & Modalities: Application of therapeutic modalities including TENS, ultrasound therapy, laser therapy, and cryotherapy as clinically indicated by the Physiotherapist.
- Documentation & Reporting: The Physiotherapist shall maintain detailed patient records in the client's electronic health system and submit a monthly progress report to the Operations Director by the fifth business day of each month.
- Staff Training & Supervision: Quarterly training workshops for the client's in-house wellness staff on basic physiotherapist techniques, injury prevention protocols, and ergonomic best practices.

Grand Total in words: Six Hundred and Three Thousand Five Hundred and Sixty-Four Chinese Yuan Only (CNY ¥603,564.00).
Payment for the services described in this Quotation Estimate shall be made in Chinese Yuan (CNY) via bank transfer to the account of Guangzhou Physiotherapy & Rehabilitation Centre Co., Ltd. The payment schedule is as follows:
- Advance Payment: 30% of the Grand Total (CNY 181,069.20) due within ten (10) business days of contract execution.
- Quarterly Installments: The remaining 70% shall be paid in four equal quarterly instalments of CNY 105,623.70 each, due on the first business day of January, April, July, and October 2025/2026.
Overdue payments shall attract a late fee of 0.05% per calendar day as stipulated under the Civil Code of the People's Republic of China.
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, prices may be revised to reflect changes in the China Guangzhou labour market, regulatory requirements, or supply costs.
- The Physiotherapist engaged under this Quotation Estimate shall comply with all applicable laws and regulations of the People's Republic of China, including the Physicians Law, the Medical Institution Administration Regulations, and local Guangdong Province health authority directives.
- Either party may terminate the service agreement with thirty (30) days' written notice. In the event of early termination, the client shall be liable for all services rendered up to the termination date plus a 10% administrative fee on the unfulfilled portion.
- All patient data and clinical records generated by the Physiotherapist shall remain the exclusive property of the client and shall be handled in strict accordance with the Personal Information Protection Law of the PRC (effective 1 November 2021).
- The Physiotherapist shall not be engaged in any concurrent employment or private practice within the Tianhe District of Guangzhou during the term of this Quotation Estimate without prior written consent from the client.
- Disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. Failing resolution within thirty (30) days, the matter shall be submitted to the Guangzhou Arbitration Commission for binding arbitration in accordance with its rules of procedure.
- This Quotation Estimate constitutes the entire agreement between the parties regarding the Physiotherapist services in China Guangzhou and supersedes all prior negotiations, representations, or understandings.
