This Quotation Estimate is issued by Addis Ababa Rehabilitation & Physiotherapy Center to provide comprehensive Physiotherapist services for the benefit of Meridian Health Insurance S.C. and its enrolled members residing in and around Ethiopia Addis Ababa. The scope encompasses outpatient physiotherapy consultations, manual therapy, therapeutic exercise programs, electrotherapy modalities, and post-surgical rehabilitation. All services will be delivered by licensed and certified Physiotherapist professionals operating within the regulatory framework established by the Ethiopian Ministry of Health and the Ethiopian Physiotherapy Association.
The Physiotherapist team assigned to this engagement will consist of a Lead Physiotherapist (Doctor of Physical Therapy), two Senior Physiotherapists, and one Junior Physiotherapist, all of whom hold valid practicing licenses issued by the Ethiopian Health Professions Authority. The services will be rendered at our primary facility located in the Bole district of Addis Ababa, with mobile outreach options available for clients in the surrounding sub-cities of Ethiopia Addis Ababa including Yeka, Nifas Silk Lafto, and Kirkos.

- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be revised to reflect changes in operational costs within Ethiopia Addis Ababa.
- Payment terms: 40% advance payment upon contract signing, 30% upon completion of the first six months of Physiotherapist service delivery, and the remaining 30% upon final acceptance and submission of the comprehensive end-of-contract report.
- All Physiotherapist services will be conducted in strict compliance with the Ethiopian Health Professions Authority regulations, the Ministry of Health clinical guidelines, and the ethical code of the Ethiopian Physiotherapy Association.
- The client reserves the right to request a minimum of two (2) on-site inspections per quarter to verify the quality and continuity of the Physiotherapist services being delivered in Addis Ababa.
- Confidentiality of all patient records and clinical data is guaranteed under the Ethiopian Data Protection Proclamation (No. 1196/2020). No patient information will be shared with third parties without written consent.
- Force majeure events, including but not limited to natural disasters, civil unrest, or government-mandated closures affecting operations in Ethiopia Addis Ababa, shall be grounds for temporary suspension of services without penalty to either party.
- Any disputes arising from this Quotation Estimate or the subsequent service agreement shall be resolved through mediation in Addis Ababa, Ethiopia, in accordance with the Ethiopian Civil Code.
- The Physiotherapist team shall maintain a minimum of 95% session attendance rate. Sessions missed due to provider illness will be rescheduled within 48 hours at no additional cost to the client.
