2. Scope of Physiotherapist Services

The Physiotherapist engaged under this Quotation Estimate shall provide the following services to the client or the client's designated personnel at the agreed-upon location within Iraq Baghdad:

Initial Comprehensive Assessment: A full musculoskeletal, neurological, and cardiopulmonary evaluation conducted by a licensed Physiotherapist holding a valid Iraqi medical license. This includes patient history review, range-of-motion testing, strength assessment, gait analysis, and functional capacity evaluation.
Therapeutic Exercise Programs: Design and supervision of individualized exercise regimens targeting post-operative recovery, sports injury rehabilitation, chronic pain management, and post-stroke mobility restoration.
Manual Therapy & Mobilization: Joint mobilization techniques, soft tissue massage, myofascial release, and therapeutic stretching performed by the Physiotherapist in a clinical setting in Baghdad.
Electrotherapy & Modalities: Application of TENS, ultrasound therapy, interferential current, cryotherapy, and heat therapy as clinically indicated.
Hydrotherapy & Aquatic Rehabilitation: Supervised pool-based exercises at the center's hydrotherapy facility located in the Al-Mansour district of Iraq Baghdad.
Home & Workplace Visits: On-site Physiotherapist visits to residential or corporate locations within Baghdad city limits, subject to travel surcharges as itemized below.

4. Payment Terms & Schedule

This Quotation Estimate for Physiotherapist services in Iraq Baghdad shall be settled as follows:

Advance Payment: 30% of the Grand Total (USD 1,310.40) due upon acceptance of this Quotation Estimate and prior to the commencement of services.
Mid-Term Payment: 40% of the Grand Total (USD 1,747.20) due upon completion of the 10th treatment session.
Final Payment: 30% of the Grand Total (USD 1,310.40) due upon completion of the final session and submission of the closing physiotherapy report.

All payments shall be made in United States Dollars (USD) via bank transfer to the account of Al-Rafidain Rehabilitation & Physiotherapy Center, or in Iraqi Dinars (IQD) at the prevailing Central Bank of Iraq exchange rate on the date of invoicing. Late payments exceeding 15 days shall incur a penalty of 2% per month.

5. Terms & Conditions

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, pricing may be subject to revision based on market conditions in Iraq Baghdad.
The Physiotherapist assigned to this engagement shall hold a minimum of a Bachelor's degree in Physiotherapy from an Iraqi-accredited university and a valid practicing license issued by the Iraqi Ministry of Health. A copy of the license shall be provided upon request.
All services shall be delivered in accordance with the ethical code of the Iraqi Physiotherapy Association and applicable healthcare regulations in Baghdad, Iraq.
The client agrees to provide a safe and accessible environment for on-site Physiotherapist visits within Iraq Baghdad, including adequate space for equipment setup and patient mobility.
Cancellation of a scheduled session must be communicated at least 24 hours in advance. Sessions cancelled with less than 24 hours' notice shall be charged at 50% of the session fee.
The Physiotherapist reserves the right to modify the treatment plan based on clinical progress, and such modifications shall be communicated to the client in writing within 48 hours.
Liability: Al-Rafidain Rehabilitation & Physiotherapy Center shall not be held liable for adverse outcomes resulting from the client's failure to follow prescribed home exercise programs or medical advice provided by the Physiotherapist.
Confidentiality: All patient records, assessment data, and progress reports generated under this Quotation Estimate shall be treated as strictly confidential in compliance with Iraqi data protection guidelines.
Disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. In the event of unresolved disputes, matters shall be referred to the competent courts in Baghdad, Iraq.
This Quotation Estimate constitutes the entire agreement between the parties regarding the scope, cost, and terms of the Physiotherapist services described herein and supersedes all prior discussions, whether oral or written.

6. Acceptance & Authorization

By signing below, the client acknowledges receipt of this Quotation Estimate for Physiotherapist services in Iraq Baghdad and agrees to the terms, conditions, and pricing outlined in this document.
