This Quotation Estimate document is issued by Mindful Path Psychological Consultancy (Pvt) Ltd. to provide a comprehensive, itemized breakdown of professional fees, session costs, and associated charges for the engagement of a licensed Psychologist to deliver clinical and organizational psychological services within the greater Sri Lanka Colombo metropolitan area. This estimate is prepared in accordance with the fee structures recommended by the Sri Lanka Psychological Association and the Sri Lanka Institute of Mental Health, and it reflects current market rates applicable to qualified psychological practitioners operating in Colombo as of the date of issue.

The Psychologist engaged under this Quotation Estimate shall provide the following services to the client, all conducted in compliance with the Sri Lanka Mental Health Act (No. 14 of 2021) and the ethical code of conduct of the Sri Lanka Psychological Association:

Clinical Assessment & Diagnosis: Comprehensive psychological evaluation including cognitive, emotional, and behavioral assessments using standardized instruments (e.g., MMPI-2, WAIS-IV, BDI-II) administered in the Colombo office or at the client's designated location within Sri Lanka Colombo.
Individual Psychotherapy: One-on-one therapeutic sessions (Cognitive Behavioral Therapy, Psychodynamic Therapy, or EMDR as clinically indicated) conducted by the assigned Psychologist.
Group Counseling & Workshops: Facilitation of group therapy sessions, stress-management workshops, and mental-health awareness programs for corporate clients or community groups in Colombo.
Organizational Consultancy: Workplace mental-health audits, employee assistance program (EAP) design, and burnout-prevention strategies tailored to the Sri Lanka Colombo business environment.
Psychological Reporting: Preparation of detailed written reports, fitness-for-work assessments, and medico-legal psychological opinions as required by the client or relevant Sri Lankan authorities.
Telehealth / Remote Sessions: Video-conferencing-based therapy sessions for clients who prefer remote engagement, available throughout Sri Lanka with primary coordination from Colombo.

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Any engagement commenced after the expiry date shall be subject to a revised fee schedule reflecting current market conditions in Sri Lanka Colombo.
A minimum advance payment of fifty percent (50%) of the total estimated cost is required to confirm the booking and secure the Psychologist's schedule. The remaining balance shall be payable within fourteen (14) days of service completion or upon receipt of the final invoice, whichever is earlier.
All payments shall be made in Sri Lankan Rupees (LKR) via bank transfer to the consultancy's designated account at Commercial Bank of Ceylon, Colombo branch, or by certified cheque. International clients may remit in USD at the prevailing Central Bank of Sri Lanka exchange rate on the date of payment.
The assigned Psychologist is a fully registered practitioner with the Sri Lanka Psychological Association and holds a valid license to practice clinical psychology in Sri Lanka. The Psychologist shall maintain strict confidentiality in accordance with the Sri Lanka Mental Health Act and professional ethical standards.
Sessions are scheduled within normal business hours (Monday to Friday, 09:00 – 17:00 Sri Lanka Standard Time) at the consultancy office on Galle Road, Colombo 03, or at the client's premises within the Sri Lanka Colombo district. After-hours or weekend sessions may be arranged at a 25% premium surcharge.
Cancellation or rescheduling of a session must be communicated at least twenty-four (24) hours in advance. Cancellations made within 24 hours or no-shows will be charged at 75% of the session fee.
This Quotation Estimate does not constitute a guarantee of specific therapeutic outcomes. The Psychologist will provide evidence-based interventions tailored to the client's individual needs, and progress will be reviewed at regular intervals.
Any additional services, extended sessions, or scope changes beyond those listed in this estimate will be subject to a supplementary quotation issued by the consultancy prior to commencement.
Disputes arising from this engagement shall be resolved through amicable negotiation. In the event of unresolved disputes, the matter shall be referred to the Colombo District Court in Sri Lanka Colombo in accordance with the laws of the Democratic Socialist Republic of Sri Lanka.
All records, reports, and session notes generated under this engagement are the property of the client and the Psychologist jointly. Reproduction or distribution of such documents requires written consent from both parties.

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and fee structure outlined above for the engagement of the Psychologist for services to be delivered in Sri Lanka Colombo. This document, once countersigned by both parties, shall serve as a binding service agreement for the scope of work described herein.
