This Quotation Estimate outlines the professional fees and associated costs for the engagement of a qualified Radiologist to provide comprehensive diagnostic imaging interpretation, consultation, and reporting services at the client's facility located in Netherlands Amsterdam. The Radiologist will operate in accordance with the Dutch Medical Treatment Act (Wgbo) and the guidelines established by the Royal Dutch Radiological Society (KNRM). All services will be delivered in compliance with the European Union Medical Devices Regulation (MDR) and the Dutch Health and Youth Care Act (Wabvz).
The Radiologist will be available on a part-time contractual basis, providing a minimum of 24 clinical hours per week across Monday through Friday, with optional Saturday morning availability for urgent contrast-enhanced studies. The engagement period covered by this Quotation Estimate is twelve (12) months, commencing on 1 September 2025 and concluding on 31 August 2026, subject to mutual renewal.

- Validity: This Quotation Estimate is valid for a period of ninety (90) days from the date of issue. After this period, all rates and terms are subject to revision based on prevailing market conditions in the Netherlands Amsterdam healthcare sector.
- Payment Terms: Invoices will be issued on a monthly basis, payable within thirty (30) calendar days of the invoice date via bank transfer to the account specified on the invoice. Late payments will incur a statutory interest rate of 8% per annum in accordance with Dutch civil law (Burgerlijk Wetboek, Book 6).
- Termination: Either party may terminate the engagement with a written notice period of sixty (60) days. In the event of termination, all services rendered up to the effective termination date shall be invoiced in full.
- Confidentiality & Data Protection: The Radiologist shall comply fully with the Dutch Implementation Act of the GDPR (UAVG) and the European General Data Protection Regulation. All patient imaging data processed in the Netherlands Amsterdam facility shall be stored on encrypted, HIPAA-equivalent compliant systems within the European Economic Area.
- Regulatory Compliance: The Radiologist holds a valid Dutch medical licence (BIG-register) and is registered with the KNRM. All diagnostic reports shall conform to the Dutch Radiology Reporting Standards and the ICD-10-CM coding system as adopted by the Netherlands.
- Force Majeure: Neither party shall be liable for failure to perform obligations due to circumstances beyond reasonable control, including but not limited to natural disasters, pandemics, or government-mandated closures affecting the Netherlands Amsterdam region.
- Governing Law: This Quotation Estimate and any resulting contract shall be governed by and interpreted in accordance with the laws of the Netherlands. Any disputes shall be submitted to the competent court in Amsterdam, Netherlands.
By signing below, the client acknowledges receipt of this Quotation Estimate for Radiologist services in Netherlands Amsterdam and agrees to the terms, conditions, and estimated costs outlined herein. This document does not constitute a binding contract until a formal service agreement is executed by both parties.
