MediScan Radiology Center, Inc. is pleased to present this Quotation Estimate for the provision of a full-time, board-certified Radiologist to support the diagnostic imaging department of St. Luke's Medical Center in the Philippines Manila metropolitan area. This Quotation Estimate outlines the comprehensive professional services, equipment utilization, and administrative support that will be rendered under a twelve-month service agreement. The Radiologist engaged under this arrangement will hold a valid Philippine Medical License (PRC), a Board Certification in Radiology from the Philippine Board of Radiology, and a minimum of ten (10) years of clinical experience in diagnostic and interventional radiology.
The Radiologist will be stationed at the client's facility in Bonifacio Global City, Metro Manila, and will be available for a standard 48-hour work week with on-call coverage for emergency imaging. All services will be delivered in strict compliance with the Philippine Data Privacy Act of 2012 (RA 10173), the Philippine Medical Technology Act (RA 5527), and the standards set forth by the Philippine Institute of Radiology.

- This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. After the expiration date, pricing may be subject to revision based on prevailing market conditions in the Philippines Manila healthcare sector.
- All amounts stated in this Quotation Estimate are in Philippine Pesos (PHP) and are inclusive of the Radiologist's professional fees, applicable government-mandated benefits (SSS, PhilHealth, Pag-IBIG), and employer's share of contributions.
- The Radiologist shall be bound by a strict Non-Disclosure Agreement (NDA) and shall comply with all patient confidentiality protocols as mandated by the Philippine Data Privacy Act of 2012.
- Payment shall be made via bank transfer to the designated account of MediScan Radiology Center, Inc. within thirty (30) days from the date of invoice issuance. Late payments shall accrue interest at the rate of 1.5% per month.
- This Quotation Estimate does not constitute a binding contract. A formal Service Agreement must be executed by both parties prior to the commencement of the Radiologist's duties.
- Any changes in scope, additional imaging modalities, or extended on-call requirements beyond those specified herein shall be subject to a supplementary Quotation Estimate issued by MediScan Radiology Center, Inc.
- The Radiologist shall report directly to the Chief of Radiology at the client's facility and shall adhere to all institutional policies, protocols, and standard operating procedures in effect in the Philippines Manila healthcare environment.
- This Quotation Estimate is non-transferable and is prepared exclusively for St. Luke's Medical Center, Bonifacio Global City, Metro Manila, Philippines.
By signing below, the parties acknowledge that they have read, understood, and agree to the terms set forth in this Quotation Estimate for Radiologist services in the Philippines Manila region. This document serves as the formal basis for the subsequent execution of the definitive Service Agreement.
