This Quotation Estimate is issued by Cape Town Advanced Radiology Centre in response to the formal request for a qualified Radiologist to provide comprehensive diagnostic imaging interpretation services for the Atlantic Medical Group outpatient facility located in the central business district of South Africa Cape Town. The following line items detail the scope of work, professional fees, and associated costs for the proposed engagement period of twelve (12) months commencing 1 August 2025.

1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Should the client wish to proceed beyond the validity period, a revised Quotation Estimate reflecting current market rates for Radiologist services in South Africa Cape Town will be issued.
2. All fees are quoted in South African Rand (ZAR) and are inclusive of the Radiologist's professional consultation, report writing, and peer-review responsibilities. Travel within the greater Cape Town metropolitan area is included; travel beyond a 50 km radius will be billed at R 8.50 per km.
3. Payment terms: 50% deposit upon contract signature, with the remaining 50% payable in equal monthly instalments over the twelve-month service period. Invoices are payable within fourteen (14) days of receipt. Late payments will attract interest at the prescribed rate as per the National Credit Act of South Africa.
4. The appointed Radiologist must hold a valid HPCSA (Health Professions Council of South Africa) registration, a South African Medical Association (SAMA) membership, and a minimum of five (5) years' post-qualification experience in diagnostic radiology. Proof of registration will be provided prior to commencement.
5. The Radiologist shall comply with all applicable South African legislation, including the National Health Act, the Protection of Personal Information Act (POPIA), and the Radiation Protection and Nuclear Safety Act. All patient imaging data will be stored in accordance with POPIA data-protection requirements.
6. Cancellation: Either party may terminate the agreement with thirty (30) days' written notice. In the event of early termination by the client, a pro-rata charge for the unfulfilled service period will apply.
7. This Quotation Estimate does not constitute a binding contract until both parties have signed and returned the acceptance section below. All services are subject to the standard terms of Cape Town Advanced Radiology Centre.
8. The Radiologist will report directly to the Chief Medical Officer of the client facility and will participate in weekly multidisciplinary team (MDT) meetings as required by the hospital's clinical governance framework in South Africa Cape Town.
