This Quotation Estimate is issued by PT Nusantara Social Services & Consulting to provide a comprehensive and transparent cost breakdown for the deployment of a qualified Social Worker to support the corporate social responsibility (CSR) and employee welfare programs of PT Artha Mandiri Group. The Social Worker will be stationed in Indonesia Jakarta and will operate in accordance with the regulations set forth by the Ministry of Social Affairs of the Republic of Indonesia (Kementerian Sosial RI), specifically under Law No. 25 of 2009 concerning Social Workers (Undang-Undang Nomor 25 Tahun 2009 tentang Pekerja Sosial).
The Social Worker engaged under this Quotation Estimate will hold a minimum of a Bachelor's degree in Social Work (S1 Ilmu Kesejahteraan Sosial) and possess a valid Social Worker Professional Certificate (Sertifikat Kompetensi Pekerja Sosial) issued by the Indonesian Social Workers Association (Ikatan Pekerja Sosial Indonesia – IPSI). The Social Worker will be responsible for conducting community outreach, employee counseling, crisis intervention, family support programs, and coordination with local government agencies across Indonesia Jakarta.

- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any acceptance beyond this period may be subject to revised pricing due to market fluctuations in Indonesia Jakarta.
- Payment terms: 50% (fifty percent) of the total Quotation Estimate amount shall be due upon contract signing, and the remaining 50% (fifty percent) shall be paid in equal monthly installments within the first five (5) business days of each calendar month.
- All payments shall be made via bank transfer to the account of PT Nusantara Social Services & Consulting, Bank Mandiri, Account No. 137-00-0456789-2, Branch: Jakarta Sudirman, Indonesia Jakarta.
- The Social Worker assigned under this engagement shall comply with all applicable Indonesian labor laws, including the Minimum Wage Regulation (Upah Minimum) for the Jakarta Special Capital Region (DKI Jakarta) as published annually by the Governor of Jakarta.
- Either party may terminate this agreement with a written notice of thirty (30) days. In the event of early termination, the client shall be liable for all services rendered up to the termination date as outlined in this Quotation Estimate.
- The Social Worker shall maintain strict confidentiality regarding all employee and client information in accordance with Indonesian data protection regulations (UU PDP No. 27 Tahun 2022).
- This Quotation Estimate does not constitute a binding contract until a formal Service Agreement is executed by both parties.
- Any disputes arising from this engagement shall be resolved through mediation in Indonesia Jakarta, in accordance with the laws of the Republic of Indonesia.
By signing below, the client acknowledges and accepts the terms, scope, and pricing outlined in this Quotation Estimate for the Social Worker services to be delivered in Indonesia Jakarta.
