Scope of Services

The following Social Worker services are proposed for delivery in Israel Jerusalem, encompassing both in-home visits and office-based consultations at our Jerusalem facility. Our Social Worker team is fully licensed by the Israeli Ministry of Welfare and Social Affairs and operates in accordance with the Social Workers Law, 5726-1966, and all relevant municipal regulations applicable to the Israel Jerusalem jurisdiction.

Terms and Conditions of This Quotation Estimate

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Upon expiration, all rates and availability of the assigned Social Worker must be reconfirmed in writing.
All Social Worker services described in this Quotation Estimate shall be performed by a licensed Social Worker holding a valid Israeli social work license issued under the Social Workers Law, 5726-1966, and registered with the Israel Social Workers Association.
The Social Worker assigned to this engagement will operate within the Israel Jerusalem municipal boundaries. Any services required outside this geographic scope will be subject to a separate supplementary Quotation Estimate.
Payment terms: 50% of the total amount is due upon acceptance of this Quotation Estimate. The remaining 50% is payable in three equal monthly installments, due on the 1st of each month, via bank transfer to the account specified by Jerusalem Community Care Services Ltd.
Cancellation Policy: The client may cancel any scheduled Social Worker session with a minimum of 48 hours' written notice. Cancellations made within 48 hours will be billed at 50% of the session fee. Full cancellation of the entire engagement after the first month will incur a 15% administrative fee on the remaining balance.
Confidentiality: All information gathered during Social Worker sessions in Israel Jerusalem shall be treated in strict accordance with the Israeli Privacy Protection Law, 5741-1981, and the professional ethical code of the Israel Social Workers Association.
This Quotation Estimate does not constitute a binding contract until signed and countersigned by both parties. The final service agreement will be executed upon acceptance.
Any changes to the scope of Social Worker services, frequency of visits, or service location within Israel Jerusalem must be documented in a written amendment to this Quotation Estimate, signed by both parties.
Disputes arising from this Quotation Estimate or the subsequent service agreement shall be governed by the laws of the State of Israel and subject to the exclusive jurisdiction of the courts in Israel Jerusalem.
The Social Worker reserves the right to refer the client to additional specialized services (psychological, medical, legal) as deemed clinically necessary, with prior written consent from the client.
