The purpose of this document is to outline the scope of therapy, the associated costs, the duration of the treatment plan, and all applicable terms and conditions governing the engagement. This Quotation Estimate is valid for a period of sixty (60) calendar days from the date of issue, after which pricing may be subject to revision based on current market rates and regulatory adjustments within Germany Frankfurt.

The Speech Therapist, Dr. Katharina Weiss, is a certified and licensed logopedist (speech and language therapist) registered with the regional health authority in Germany Frankfurt. The services outlined in this Quotation Estimate encompass a full range of diagnostic and therapeutic interventions tailored to the specific needs of the patient. The Speech Therapist will conduct an initial comprehensive assessment covering articulation, phonology, fluency, voice, language comprehension, language expression, and, where applicable, swallowing function (dysphagia). Following the assessment, a personalised therapy plan will be developed and implemented over the agreed treatment period.

All sessions will be conducted at the Praxis für Logopädie am Main clinic located in the central district of Germany Frankfurt, or alternatively at the patient's residence within the city limits of Frankfurt am Main, subject to a small travel surcharge as detailed in Section 4.

The total amount specified in this Quotation Estimate is payable in monthly instalments aligned with the therapy cycle. The first payment of 50% of the initial assessment fee is due upon confirmation of the booking. Subsequent monthly invoices will be issued on the first business day of each month and are payable within fourteen (14) calendar days via bank transfer to the account details provided on each invoice. Late payments will incur a statutory interest charge of 5 percentage points above the European Central Bank base rate, in accordance with German commercial law (BGB § 288). This Quotation Estimate does not include costs for additional diagnostic imaging, audiometric testing, or specialist referrals, which will be itemised separately if required during the course of therapy.

Appointments with the Speech Therapist may be rescheduled or cancelled without charge provided a minimum of forty-eight (48) hours' written notice is given. Cancellations made within less than forty-eight hours will be subject to a fee of 50% of the session rate. No-shows will be charged the full session fee. In the event of extended illness or force majeure circumstances, the Speech Therapist will work with the patient to adjust the treatment timeline at no additional cost, and this Quotation Estimate will be amended accordingly in writing.

All patient records, therapy notes, and personal data processed in connection with this Quotation Estimate and the subsequent Speech Therapist engagement are handled in strict compliance with the General Data Protection Regulation (GDPR) and the German Federal Data Protection Act (BDSG). No information will be shared with third parties without the explicit written consent of the patient or legal guardian, except where mandated by German law. Records are stored securely at the clinic in Germany Frankfurt and are retained for the statutory period of ten years following the conclusion of therapy.

This Quotation Estimate becomes a binding agreement upon written acceptance by both parties. The client is requested to sign and return a copy of this document within ten (10) business days to confirm the engagement of the Speech Therapist. Should the client require any modifications to the scope, schedule, or pricing outlined herein, a revised Quotation Estimate will be issued at no charge. This document is governed by the laws of the Federal Republic of Germany, and any disputes arising from this engagement shall be subject to the exclusive jurisdiction of the competent courts in Frankfurt am Main, Germany Frankfurt.
