1. Purpose of This Quotation EstimateThis Quotation Estimate is prepared by ClearVoice Speech Therapy Centre to provide Mr. Tan Wei Ming with a comprehensive and transparent breakdown of professional Speech Therapist services to be delivered within Singapore Singapore. This document outlines the scope of therapy, session frequency, associated costs, and all relevant terms governing the engagement of our licensed Speech Therapist. The Quotation Estimate is valid for thirty (30) days from the date of issue and is subject to the terms and conditions stated herein.
2. Scope of Speech Therapist ServicesOur qualified Speech Therapist, Ms. Rachel Lim (Registered with the Singapore Health Sciences Council, Registration No. HSC-2019-4471), will provide the following therapeutic interventions tailored to the client's specific communication and swallowing needs:
- Assessment & Evaluation: A comprehensive initial assessment of speech, language, voice, and fluency disorders conducted by the Speech Therapist at our Singapore Singapore facility. This includes standardised testing, clinical observation, and a detailed written report.
- Individual Therapy Sessions: One-on-one Speech Therapist sessions focusing on articulation, phonological processing, language comprehension and expression, and voice modulation as recommended in the treatment plan.
- Group Therapy Sessions: Small-group Speech Therapist sessions (maximum 4 participants) designed to enhance social communication skills, pragmatic language use, and peer interaction in a supportive environment.
- Home Programme Guidance: The Speech Therapist will provide structured home exercises and parent/caregiver training to reinforce therapeutic gains between sessions.
- Progress Review & Reporting: Monthly progress reports issued by the Speech Therapist, including updated treatment goals and recommendations for the next phase of therapy in Singapore Singapore.

* All prices are quoted in Singapore Dollars (SGD) and are inclusive of GST at 9%. The Quotation Estimate covers a twelve (12) month treatment programme. Individual therapy sessions total 96 sessions over the programme period.
4. Payment TermsThe total amount stated in this Quotation Estimate shall be payable as follows: a 20% deposit (SGD $3,994.00) is due upon acceptance of this Quotation Estimate to secure the Speech Therapist's schedule. The remaining balance shall be payable in twelve (12) equal monthly instalments of SGD $1,431.33, commencing from the first therapy session. Payments may be made via bank transfer, PayNow, or credit card. A 2% late payment surcharge will apply to any instalment not settled within fourteen (14) days of the due date.
5. Terms & Conditions5.1 This Quotation Estimate is non-transferable and is issued exclusively to the named client in Singapore Singapore.
5.2 The Speech Therapist reserves the right to adjust the treatment plan based on clinical progress. Any significant change in scope will be communicated in writing and may result in a revised Quotation Estimate.
5.3 Cancellations of individual Speech Therapist sessions must be made at least 24 hours in advance. Late cancellations or no-shows will be charged at 50% of the session fee.
5.4 All client records, assessment reports, and therapy notes are the property of ClearVoice Speech Therapy Centre and are stored in compliance with the Personal Data Protection Act (PDPA) of Singapore Singapore.
5.5 This Quotation Estimate does not constitute a guarantee of specific clinical outcomes. The Speech Therapist will exercise professional judgement in delivering evidence-based interventions.
5.6 Any disputes arising from this Quotation Estimate shall be resolved under the jurisdiction of the courts of Singapore Singapore.
6. AcceptanceBy signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined for the Speech Therapist services in Singapore Singapore. The engagement of the Speech Therapist shall commence upon receipt of the initial deposit.
