This Quotation Estimate has been prepared in response to the formal request submitted by Osaka Prefectural General Hospital for the engagement of a qualified Surgeon to provide specialized surgical services within the Japan Osaka metropolitan medical district. The scope of this estimate encompasses the full spectrum of professional fees, operational costs, and ancillary charges associated with the deployment of a board-certified Surgeon for a continuous engagement period of twelve (12) months, commencing 1 September 2025.
The Surgeon referenced in this Quotation Estimate shall hold a valid Japanese Medical Practitioner License (Iryō Ijin Shō), a specialist certification in General Surgery or Vascular Surgery issued by the Japanese Medical Association, and a minimum of fifteen (15) years of post-graduate surgical experience. All services shall be rendered in compliance with the regulations set forth by the Osaka Prefectural Board of Health and the Ministry of Health, Labour and Welfare of Japan.

The Surgeon engaged under this Quotation Estimate must satisfy the following minimum criteria: (a) a Doctor of Medicine (M.D.) degree from an accredited Japanese or internationally recognized medical institution; (b) completion of a five-year residency in General Surgery at a designated training hospital in Japan; (c) a minimum of fifteen years of active surgical practice, with documented performance of no fewer than 200 major surgical procedures per annum; (d) current registration with the Osaka Prefectural Medical Association; and (e) proficiency in both Japanese and English for patient communication and multidisciplinary team coordination within the Japan Osaka healthcare network.
4.1 This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, all rates and conditions are subject to revision without prior notice.
4.2 Payment terms: 50% of the monthly Surgeon professional fee shall be due within fifteen (15) days of the invoice date. The remaining 50% shall be settled by the end of the following calendar month. All payments shall be made via bank transfer to the designated account of the Osaka Prefectural General Hospital.
4.3 The Surgeon shall be subject to the employment and labor laws of Japan, including the Labor Standards Act and the Medical Practitioners Act. All social insurance contributions (health insurance, pension, and employment insurance) shall be handled in accordance with Japan Osaka municipal regulations.
4.4 This Quotation Estimate does not constitute a binding contract. A formal Service Agreement must be executed by both parties prior to the commencement of any Surgeon services. The terms herein are indicative and subject to final negotiation.
4.5 Any additional surgical procedures beyond the scope defined in Section 2 shall be billed separately at the prevailing Japan Osaka medical fee schedule (Shinryō Hikin Jōkyō) as published by the Ministry of Health, Labour and Welfare.
4.6 The Surgeon shall maintain a minimum of 1,200,000 JPY in professional liability insurance coverage throughout the engagement period, with the hospital named as an additional insured party.
4.7 Termination: Either party may terminate the engagement with a written notice of sixty (60) days. In the event of termination, all outstanding fees for services rendered up to the termination date shall be settled within thirty (30) days.
