This Quotation Estimate is formally issued by Lagos Surgical Excellence Centre to provide a comprehensive, transparent, and itemised financial breakdown for the engagement of a qualified Surgeon and associated surgical services to be delivered within the Nigeria Lagos metropolitan area. All pricing in this Quotation Estimate is denominated in Nigerian Naira (NGN) and reflects current market rates as of the date of issuance in Lagos, Nigeria.
The Surgeon engaged under this Quotation Estimate shall provide the following professional services at our fully accredited surgical facility located in Victoria Island, Lagos, Nigeria. The Surgeon is a board-certified specialist registered with the Medical and Dental Council of Nigeria (MDCN) and holds a minimum of fifteen (15) years of post-qualification surgical experience in general, vascular, and minimally invasive surgical procedures.

The Surgeon designated for this engagement holds the following credentials: Bachelor of Medicine and Bachelor of Surgery (MBBS) from the University of Lagos; Membership of the Surgical Society of Nigeria (MSSN); Fellowship of the West African College of Surgeons (FWACS); and a subspecialty certification in Minimally Invasive Surgery. The Surgeon is currently practising at a Level 5 hospital in Nigeria Lagos and maintains full professional indemnity insurance coverage through the Medical Defence Association of Nigeria (MDAN). All surgical procedures will be conducted in strict compliance with the standards set by the National Health Act 2014 and the MDCN Code of Medical Ethics.
4.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. Beyond this period, all prices are subject to revision due to fluctuations in the Nigerian Naira exchange rate, changes in pharmaceutical pricing, or updates to the National Health Insurance Authority (NHIA) fee schedule.
4.2 Payment Terms: A non-refundable deposit of fifty percent (50%) of the total Quotation Estimate amount is required upon acceptance to secure the Surgeon's schedule and reserve the operating theatre. The remaining fifty percent (50%) shall be settled within seventy-two (72) hours of the completion of the surgical procedure. Payment may be made via bank transfer to our designated account at Zenith Bank, Lagos Branch, or by certified bank draft.
4.3 Cancellation Policy: Cancellation by the client more than fourteen (14) days prior to the scheduled surgery date shall incur a fee of twenty percent (20%) of the total Quotation Estimate. Cancellation within fourteen (14) days shall incur a fee of fifty percent (50%). Cancellation within seventy-two (72) hours of the scheduled procedure shall result in forfeiture of the full deposit.
4.4 Insurance and Liability: This Quotation Estimate does not include health insurance premiums. The client is advised to confirm coverage with their NHIA-accredited insurer prior to the procedure. The Surgeon and Lagos Surgical Excellence Centre shall not be held liable for complications arising from pre-existing conditions not disclosed during the pre-operative consultation.
4.5 Confidentiality: All patient records, surgical reports, and medical data generated under this engagement shall be treated as strictly confidential in accordance with the Nigeria Data Protection Act 2023 (NDPA). No information shall be disclosed to third parties without the written consent of the patient or their legal representative.
4.6 Governing Law: This Quotation Estimate and any disputes arising therefrom shall be governed by the laws of the Federal Republic of Nigeria. Any unresolved disputes shall be referred to arbitration in Lagos, Nigeria, in accordance with the Arbitration and Conciliation Act 2023.
Important Note: This Quotation Estimate is a financial proposal and does not constitute a medical diagnosis or treatment plan. The final surgical approach will be determined by the Surgeon following the pre-operative clinical assessment. Additional costs may arise if intra-operative findings necessitate extended procedures, additional implants, or prolonged hospitalisation. Any such additional costs will be communicated to the client or their authorised representative prior to commencement and will be itemised in a supplementary Quotation Estimate.By signing below, the client acknowledges receipt of this Quotation Estimate, confirms understanding of all terms and conditions outlined herein, and authorises Lagos Surgical Excellence Centre to proceed with the engagement of the Surgeon and the delivery of the surgical services described in this document within the Nigeria Lagos area.
