This Quotation Estimate is issued by MediCare Surgical Services (Pvt.) Ltd., a registered medical staffing and surgical consultancy firm headquartered in Pakistan Islamabad, in response to the formal request received from Shifa International Hospital for the engagement of a qualified and experienced Surgeon to perform general surgical, laparoscopic, and emergency surgical procedures at their Islamabad campus. The Surgeon to be engaged under this Quotation Estimate shall be a Board-Certified General Surgeon with a minimum of fifteen (15) years of post-qualification experience in general surgery, laparoscopic surgery, and emergency trauma care. The Surgeon shall hold valid registration with the Pakistan Medical Council (PMC) and shall possess a current Practicing Certificate valid throughout the engagement period. The services shall be rendered exclusively at the designated surgical theatre and associated wards within the hospital premises located in Pakistan Islamabad.
The scope of the Surgeon's duties includes but is not limited to: performing elective and emergency general surgical procedures (appendectomy, cholecystectomy, hernia repair, bowel resection, and thyroidectomy); conducting laparoscopic and minimally invasive surgical interventions; providing on-call emergency surgical coverage for a period of twenty-four (24) hours per day, seven (7) days per week; supervising and mentoring junior surgical residents and registrars; participating in multidisciplinary surgical case conferences; and maintaining complete and accurate operative notes and patient records in compliance with the Pakistan Medical Council's ethical and documentation standards.

Payment against this Quotation Estimate shall be made in monthly installments. The client shall remit the monthly professional fee, on-call allowance, and supervisory fee within fifteen (15) calendar days from the end of each calendar month via bank transfer to the designated account of MediCare Surgical Services (Pvt.) Ltd., National Bank of Pakistan, Blue Area Branch, Pakistan Islamabad. Procedure-specific surcharges (laparoscopic and emergency) shall be invoiced separately within five (5) working days of each procedure and shall be payable within ten (10) working days of invoice receipt. A late payment penalty of 1.5% per month shall apply to any overdue amounts.
- This Quotation Estimate is valid for thirty (30) days from the date of issue. Beyond this period, all rates and terms are subject to revision without prior notice.
- The Surgeon engaged under this arrangement shall be bound by the Pakistan Medical Council's Code of Professional Conduct and Ethics, as well as the internal policies and protocols of the client hospital in Pakistan Islamabad.
- MediCare Surgical Services (Pvt.) Ltd. shall be responsible for the recruitment, credentialing, and initial vetting of the Surgeon, including verification of PMC registration, post-graduate qualifications, and references from previous surgical institutions.
- The client hospital shall provide all necessary surgical instruments, consumables, anaesthesia support, nursing staff, and theatre facilities required for the Surgeon to perform procedures safely and in accordance with international surgical standards.
- Either party may terminate this engagement with a written notice of thirty (30) days. In the event of termination, all fees accrued up to the date of termination shall remain payable.
- All disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. Failing resolution, the matter shall be referred to arbitration in Pakistan Islamabad in accordance with the Arbitration Act, 1940 of Pakistan.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by authorized representatives.
- All personal health information and patient data handled by the Surgeon shall be treated with strict confidentiality in compliance with the Pakistan Personal Data Protection Bill and institutional data privacy policies.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and cost structure outlined herein for the engagement of the Surgeon at the specified facility in Pakistan Islamabad.
