Bhd. in accordance with the signed Actor Service Agreement (Ref: GHP-CTR-2025-0312) governing the engagement of the named Actor for the production "Echoes of the Straits." All services were rendered in and around Malaysia Kuala Lumpur during the period specified above.
- Payment is due within fifteen (15) calendar days from the date of this Invoice, i.e., on or before 30 June 2025.
- Remittance shall be made via bank transfer to: Maybank Berhad, Account Name: Starlight Performers Agency Sdn. Bhd., Account No: 5123-4567-8901, Branch: Malaysia Kuala Lumpur (01400).
- Please quote Invoice No. SLPA-2025-00847 in the payment reference field to ensure proper reconciliation.
- The Service Tax (SST) of 8% has been applied as mandated by the Royal Malaysian Customs Department for entertainment and performance services rendered in Malaysia Kuala Lumpur.
- The PCB (Pajak Cukai Bulanan) deduction of 5% has been withheld as required under Section 109 of the Income Tax Act 1967 for payments to resident individuals engaged as an Actor.
- Any dispute regarding this Invoice must be raised in writing within seven (7) days of receipt. Failure to do so shall constitute acceptance of the charges listed herein.
- This Invoice is valid for a period of ninety (90) days from the date of issue. After this period, a late payment surcharge of 2% per month will be applied.
