Late Fees: In accordance with standard business practices in the United States, a late fee of 1.5% per month will be applied to any balance remaining unpaid after the due date of November 23, 2023.

Scope of Work: This Invoice reflects the professional services rendered by a certified Biomedical Engineer. All services were performed in strict adherence to the engineering standards set forth by the National Society of Professional Engineers (NSPE) and relevant FDA guidelines applicable to medical device maintenance and operation in Chicago, Illinois.
