- This invoice is issued by MediChem Bangalore Pvt. Ltd., a registered and licensed chemist operating under the Drugs and Cosmetics Act, 1940, and the Karnataka State Pharmacy Council regulations. All pharmaceutical products listed herein are sourced from approved manufacturers and distributed in compliance with the Central Drugs Standard Control Organisation (CDSCO) guidelines applicable in India.
- Payment is due within 15 days from the date of this invoice. Late payments shall attract a penalty of 1.5% per month as per the standard commercial terms agreed upon between the chemist supplier and the purchasing institution in Bangalore, Karnataka.
- All Schedule H and Schedule H1 medicines listed in this invoice have been dispensed and supplied in strict accordance with the prescription and authorization provided by the registered medical practitioner at the receiving hospital. The chemist assumes no liability for misuse beyond the prescribed dosage.
- Cold chain products (Item No. 8 – Insulin Glargine) must be stored at 2°C to 8°C upon receipt. The chemist's logistics partner in Bangalore ensures temperature-controlled transit. Any damage or temperature excursion must be reported within 24 hours of delivery to initiate a replacement claim.
- GST has been calculated as per the Central Goods and Services Tax Act, 2017, and applicable Karnataka state regulations. The GSTIN of the chemist supplier is printed on this invoice for the purpose of input tax credit claims by the buyer. All tax invoices are generated through the chemist's Tally ERP 9 software registered with the Bangalore GST portal.
- Disputes arising from this invoice shall be subject to the exclusive jurisdiction of the courts in Bangalore (Bengaluru), Karnataka, India. No part of this invoice or the associated supply agreement may be modified without written consent from both parties.
- This invoice is valid for a period of 30 days from the date of issue. After this period, the chemist reserves the right to revise pricing based on current market rates in the Bangalore pharmaceutical trade.
